Responsibilities
Job Summary / Purpose
The Patient Navigator will act as the key non-clinical contact that identifies needs and provides information about services and processes.
Essential Key Job Responsibilities
- Guide patients through and around barriers within the healthcare system.
- Maintaining communication with patients, families, and health care providers to monitor patient satisfaction.
- Direct patients to resources and manage information.
- Coordinating screenings and appointments.
- Arranging transportation.
- Facilitating follow-up services.
- Facilitating financial support and helping with paperwork.
- Perform other duties as assigned.
Qualifications
Required Education and Experience
Bachelor’s Degree
Two years of related experience in a healthcare environment.
Required Licensure and Certifications
None
Required Minimum Knowledge, Skills and Abilities
Strong customer service skills.
Knowledge of medical terminology.
Pay Range
$15.61 - $22.34 /hour
Salary : $16 - $22
