Position Summary
This grant-funded position results from a collaboration between Chicago Commons and the Center for Better Aging. The Licensed Practical Nurse will oversee case management responsibilities for the Welcome Home program, including conducting initial assessments and making referrals for clients discharged from the St. Benard Emergency Room.
In accordance with the Nurse Practice Act and regulatory standards, the LPN will provide patient care and supervise non-clinical staff to ensure effective case management services. This includes utilizing assessment tools, making necessary referrals, coordinating services, and advocating for clients to enhance their health outcomes.
As a home care nurse, this position requires strong clinical assessment, critical thinking, communication, and interpersonal skills. Travel to client’s home is necessary to fulfill job duties.
Education & Certification Requirements
- A current Illinois licensed practical nurse.
- Minimum of 5 years of nursing experience in a clinical setting.
- Prior home care experience preferred.
- Graduate of a state approved nursing school.

Essential Job Requirements & Duties
1. Makes initial evaluation visits to evaluate client’s needs.
2. Initiates and coordinates program services for clients.
3. Attend and participate in clinical huddles and meetings related to the program.
4. Supervise non-clinical staff within the program.
5. Initiate appropriate preventive services as necessary for clients.
6. Maintain patient assessments and confidentiality in accordance with HIPAA regulations.
7. Facilitates regular meetings with program staff.
8. Promote, and comply with safety standards in a client's home.
9. Ensures all documentation is consistently legible, accurate, and completed in a timely manner.
10. Complies with Chicago Commons handbook, policies and procedures.
11. Consults with a Registered Nurse to ensure compliance and regulatory standards.
12. Attends all compulsory and recommended training and meetings.
13. Provide education to patients and their families.
14. Performs data collection as directed for trend analysis and program development.
15. Serves as a liaison between clients and external agencies.
16. Performs additional duties commensurate with job tasks

The employee frequently is required to stand, walk, reach with hands and arms, and stoop, kneel, crouch, or crawl. The employee must be able to lift and / or move up to 20 pounds occasionally. Specific vision requirements include the ability to see at a close range.
