We are seeking a highly motivated and experienced Lead Provider to join our team. As the Lead Provider, you will be responsible for collaborating with the Clinic Manager to act as the site management team for the respective clinical site. You will also act as a champion for guiding and educating providers to attain excellence in clinical care. Leverages industry knowledge and best practices to promote the mission and vision of the organization as well as supporting a culture of development.

Lead Responsibilities:
In collaboration with the Clinic Manager, the Lead Provider will act as site management team for respective clinical site.
Act as a champion for guiding and educating providers to attain excellence in clinical care.
Responsible for leading monthly Providers Meeting at their respective Clinic and as a monthly rotation basis for the organization wide Providers Meeting.
Support the CMO in Provider Chart Audits to support Quality Improvement, Quality Assurance and Peer Review processes and other compliance matters.
Chart review for new clinic site providers as per new provider onboarding.
Collaborate with Clinic Manager to ensure timely chart completion of the provider teams.
Chart review as needed for clinic site providers or other CFHC providers.
Ensure that providers are seeing appropriate patients for their level of training and experience.
Collaborate with respective Clinic Manager to help coordinate time off requests and delegate clinical coverage during provider time off. The goal is to always have at least two (2) providers in the clinic.
Make timely and appropriate recommendations for improvements in clinic infrastructure and staffing, assist in provider recruitment for site provider vacancies.
General Provider Responsibilities:
Performs medical examinations, procedures, and prescribes medications to patients as assigned or as needed.
Provides patients with health education and ensures patients receive educational materials that pertain to their health needs.
Refers patients to specialists or referral services when required and in accordance with their insurance restrictions.
Ensures Integrated Care Model with patients in need of behavioral health services or dental services are routed to the behavioral health or dental department.

Requirements:
Education and Experience
Doctor of Medicine (M.D.)/ Doctor of Osteopathic Medicine (D.O.)
Board-certified or Board-eligible in Family Practice or Internal Medicine or another approved specialty.
Licensed in Internal Medicine, Family Medicine, OB/GYN or Pediatrics
Certifications and/or licensures required (if applicable):
Current and valid DEA Controlled Substance Registration Certificate
Current and valid Illinois license as a physician and surgeon (Physician)
Knowledge/Skills/Abilities
Demonstrated ability to collaborate effectively in a team setting.
Ability to maintain effective and professional relationships with patients and other members of the care team.
Comfort working in a medical setting as part of a multi-disciplinary team.
Ability to observe, assess, and record symptoms, reactions, and progress
Knowledge of legal and ethical standards for the delivery of medical care
Knowledge of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosages and instruct patients in correct usage
Knowledge of related accreditation and certification requirements
Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise
Experience in working with a diverse population base preferred.
Ability to appropriately handle confrontation and conflict.
Ability to work independently and to seek consultation as necessary.
Ability to follow organization and departmental processes and procedures.
Ability to work in a high stress/fast paced/high work volume environment.
