OBJECTIVE AND POSITION SUMMARY
The Health Care/Behavioral Services Case Manager at Gateway West Homeless Shelter is responsible for coordinating and delivering health care and behavioral case management services to clients experiencing homelessness. This role involves conducting health and mental health assessments, developing individualized care plans, and ensuring clients have access to medical, behavioral health, housing, and social services. The Case Manager collaborates with a multidisciplinary team to promote client well-being, stability, and long-term health outcomes, addressing the unique challenges faced by individuals in a homeless shelter environment.
SPECIALIZATIONS
- Proficiency in case management practices, including both health care and behavioral health assessment and care planning tailored to homeless populations
- Knowledge of trauma-informed care principles, health care systems, and behavioral health services
- Experience in care coordination across multiple healthcare providers, behavioral health specialists, and support systems
- Strong understanding of the unique health challenges and barriers to care faced by individuals experiencing homelessness
- Ability to facilitate medical and mental health crisis intervention and stabilization
- Familiarity with local healthcare, behavioral health, housing, and social service resources relevant to homeless populations
- Ability to maintain accurate and timely documentation of health care and behavioral interventions
MINIMUM QUALIFICATIONS AND COMPETENCIES
Education /Background:
- Bachelor’s Degree, preferred or
- Two years of experience in housing, health care/behavioral health case management or social service delivery
- A combination of a minimum of six years of work experience and education will be considered
- At least two years of experience in case management within healthcare or behavioral health settings, preferably with clients experiencing homelessness
- Strong understanding of trauma-informed care principles and a commitment to applying them in practice
- Ability to work effectively with diverse populations and demonstrate cultural competency
- Excellent communication, problem-solving, and organizational skills
- Ability to maintain professionalism and confidentiality in a fast-paced, high-stress environment
- Proficiency in basic computer applications, including Microsoft Office and client management software
- Valid driver’s license and ability to travel locally as needed
