Job Summary: The Housing Navigator/Community Support Provider is responsible for navigating referrals from community partners and assisting in the placement of residents. This position is essential in meeting our organization’s mission of providing our clients with stable, safe, and effective housing opportunities.
This position handles the intake of new residents and housing eligibility for CHAT's various programs (50% of the time) and conducts monthly resident check-ins to ensure their housing needs are met; determining their continued eligibility for CS services. (50%)
Job Duties:
- Maintain a working knowledge of CHAT programs and program eligibility and work with
program administrator to ensure residents are selected appropriately.
- Maintain awareness of fair housing regulations.
- Connect with agencies to assist in resident selection.
- Perform duties as they pertain to CalAIM Community Supports; assisting with intake and monthly check-ins.
- Accurately document work/contacts made with residents utilizing appropriate databases according to industry regulations and CHAT expectations.
- Build and sustain collaborative partnerships with agencies and county programs to coordinate efforts in housing individuals.
- Continuously research and stay informed about local resources to bridge gaps and reduce barriers to CHAT housing for referrals.
- Maintain a thorough understanding of CHAT's properties to effectively match individuals and families with housing options that meet their specific needs.
- Collaborate with HSCs to ensure they remain aligned and on schedule during the resident onboarding process.
- Support current residents in transitioning from CHAT housing by connecting them with local resources and affordable housing options.
- Support the Social Services team in facilitating conflict resolution among residents.
- Navigate and utilize multiple software platforms with efficiency and accuracy.
- Work independently and productively while still keeping open communication with others.
- Deliver honest and reliable communication with all individuals with whom they interact.
- Effectively manage time to avoid backtracking and delay of tasks.
- Consistently contribute to a safe and positive work environment, abiding by the policies and following procedures at all times.
- Maintain regular attendance and timeliness.
- Perform other duties as assigned or otherwise identified.
Knowledge, Competencies, Skills, and Qualifications:
- Knowledge of grant/program requirements (required)
- Bachelor's Degree (preferred)
- AA Degree from an accredited institution (preferred)
- Experience with Housing Community Partners (preferred)
- Experience with CalAIM funding (preferred)
- Experience with vulnerable populations (preferred)
- Familiarity with Partnership Health Plan and using their portal (preferred)
- Current driver’s license in order to drive to locations while on duty (required)
- Ability to learn and navigate multiple software programs such as Clarity, the county HMIS (homeless management information system) and our property management software
- Ability to build relationships: Establish and maintain positive working relationships with others, both internally and externally, to achieve the goals of the organization. (required)
- General understanding of landlord-tenant law, including proper service of documents. (preferred)
- Knowledge of Sec. 8 rules and requirements. (preferred)
- General knowledge of home inspection and ability to identify home repair needs. (required)
- Proven ability to foster teamwork: Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness with an appreciation of CHAT’s unique participatory culture. (required)

Typical Working Environment:
Work is performed primarily indoors. Time is spent primarily in an office setting with moderate noise levels consistent with a typical office setting.
Equipment/Tools used: Computer, Keyboard, Phone, Tablet/iPad, Multifunction Printer/Copier, and other miscellaneous office equipment.
Interaction/Supervision: Interact with all levels of staff, vendors, property managers, vendors, residents, families, community, and partners.
Physical Demands:
Constantly (67% ): Talking, hearing, walking on normal surfaces, and using a keyboard.
Frequently (34-66%): Sitting, repetitive hand motion.
Occasionally (1% - 33%): Lifting and carrying up to 20 lbs, standing, walking on uneven surfaces, grasping/holding, fine dexterity with hands, feeling.
Rarely (less than 1 hour per week): Reaching outward, reaching overhead, bending.

Pay: $23.00 - $27.00 per hour
Expected hours: 40 per week
