What are the responsibilities and job description for the Service Coordinator position at Child Adult Resource Services?
The Service Coordinator is responsible for managing all needed supports for individuals on their caseload, including hiring, training and supervision of Waiver Direct Support Professionals who provide direct services to the individuals. Duties not limited to but include
- Bachelor’s Degree (BA/BS) in Human Services, Education, Special Education and/or related field; number years of experience in the field may substitute for education. Experience in teaching and/or management with an emphasis on individualized instruction or a background in Social Services. Experience with individuals with ID/DD is preferred Provide for quality programs and services, ethical treatment of individuals, and protection of their rights and dignity as persons.
- Coordinate, schedule and participate in all quarterly / annual PCISP review meetings for individuals on caseloads
- Prepare appropriate progress information, forms and assessments prior to the PCISP meeting(s).
- Assist in developing the individual’s PCISP-Person Centered Individual Support Plan according to established Federal, State and Agency guidelines.
- Advocate for the individuals on caseload and their family / caregivers.
