Salary Range (final salary offer is based on a number of factors, such as certifications, experience, and more):
$23.08 - $25.10/hour

MINIMUM QUALIFICATIONS:
- Bachelor of Art or Science Degree in Human Services, Social Work or Psychology or three years’ experience working with high-risk children/youth in either child welfare, probation or mental health systems in Wraparound.
- Must possess a valid California driver's license and maintain an insurable driving record under the Clinics' liability policy. May be required to obtain a Class II California driver’s license.
- Bilingual in Spanish required to meet the client's needs.
- Must pass all applicable health, fingerprint/LiveScan and DMV clearances.
JOB PURPOSE:
The Facilitator - Wraparound (WRA) promotes the mission of the Child & Family Center by ensuring that the WRA team provides services in accordance with the WRA model and philosophy and in keeping with all Center policies and procedures. The Facilitator serves as the primary contact point for children and families in the WRA program, other service providers and other community partners and members.
ESSENTIAL JOB FUNCTIONS:
1. Act as leader of individual WRA Child & Family Team.
2. Convene the team and ensure continuity of care.
3. Ensure that all identified services are provided in a timely and appropriate manner.
4. Ensure application of family-centered practice by the entire team.
5. Serve as contact point for children and their families.
6. Serve as contact point for service providers and the community.
7. Ensure continuous input and feedback from the family and service providers.
8. Attend all training meetings as assigned.
9. Provide individual rehab and linkage services as needed.
10. Meet established documentation and productivity expectations.
11. Be available and responsive by agency cell phone for crisis management.
12. All other duties as assigned by Supervisor.
13. This is a field-based position, as the majority of the services are provided in the field.
WORK SCHEDULE: (subject to change based on Center’s needs)
Includes some on-call time after-hours.
FLSA Status: Non-Exempt – Full Time
Work-Week: Sunday - Saturday
