GENERAL DESCRIPTION :

The Program Supervisor is responsible for the supervision of Family Support Specialists and effectively collaborating with the other program staff from collaborating agencies. This position reports directly to the Senior Program Supervisor.

The Family Assessment Worker is responsible for initial identification, referral and coordination activities for families of newborns. This position reports directly to the Family Assessment Supervisor.

SUMMARY OF ESSENTIAL JOB FUNCTIONS :

  • Coordinates, supervises and participates in all activities of the Healthy Families program.
  • Provides support to staff and participates in daily activities essential to the completion of program objectives; including, but not limited to, family assessment, home-based family support, referral and coordination, case plan development, and documentation of services.
  • Provides strength-based, reflective supervision with each staff member on a weekly basis according to contract requirements.
  • Maintains and monitors staff caseloads and work assignments.
  • Evaluates program areas and service needs on a regular basis according to agency, funding, and quality assurance standards.
  • Evaluates staff performance on a regular basis, giving feedback at least quarterly and formal evaluations once a year.
  • Meets regularly with staff team to coordinate activities, address issues, and review program goals and objectives.
  • Ensures that necessary data is collected and provided to program evaluators and the Program Director in a timely manner.
  • Regularly attends and actively participates in the collaborative supervisor team meetings and supports all collaborative efforts.
  • Ensures that the Healthy Families statewide accreditation is maintained.
  • Coordinates program activities with hospital, Child Protective Services, community-based agencies and the public.
  • Attends and participates in all required Healthy Families seminars and in service trainings.
  • Performs other related duties as required.
  • Conducts FROG assessments virtually or in-person.
  • Provide direct support to prenatal or postpartum women identified as high risk.
  • Receive daily referrals from hospital or clinic personnel.
  • Administer the Healthy Families Screen on all referrals to determine risk level and need for early intervention services.
  • Obtain permission for Family Support Staff to visit the home.
  • Ensure referral to the Family Support Specialist for continued follow-up and support services.
  • Maintain client records in accordance with program and funding source requirements.
  • Meet weekly with Senior Program Supervisor to discuss and review each referral.
  • Attend and participate in all scheduled Healthy Families seminars, conferences and in-service trainings.
  • Make DCS referrals as appropriate.
  • Participates in agency Quality Assurance activities.

REQUIREMENTS :

  • Job Title : Program Supervisor / Family Assessment Worker (.75 / .25 FTE Split)
  • Program : Healthy Families
  • Location : Pima- Tohono O’odham Nation, Sells AZ
  • Master’s degree in human services or fields related to working with children and families, or bachelor’s degree in these fields with 3 years of relevant experience, or less than a bachelor’s degree but with commensurate HFA experience
  • A solid understanding of or experience in supervising diverse staff with humility, as well as providing support to staff in stressful work environments
  • Knowledge of infant and child development and parent-child attachment experience with family services that embraces the concepts of family-centered and strength-based service provision
  • Ability to mentor, supervise and support staff.
  • Knowledge of parent-infant health and dynamics of child abuse and neglect
  • Experience supporting culturally diverse communities / families
  • Willingness to engage in Reflective Practice
  • Experience with reflective practice preferred
  • Experience in home visiting with a strong background in early childhood prevention services
  • Infant Toddler Mental Health Endorsement preferred
  • Flexibility in scheduling (able to work limited nights and weekends).
  • Ability to work effectively within a team.
  • Knowledge of community resources and existing community prevention efforts.
  • Ability to communicate effectively both orally and in writing.
  • Demonstrates effective problem-solving skills.
  • Grant writing abilities preferred.
  • Adheres to agency policies & procedures
  • Meets department performance / productivity standards
  • Must be eligible and able to obtain a fingerprint card.
  • Must be certified in CPR / First Aid
  • Must have valid AZ driver's license, AZ insurance and vehicle registered in state of AZ.

    ABILITY REQUIREMENTS :

  • Sitting for extended periods of time daily; standing and walking short distances.
  • Moderate twisting, bending, stooping and lifting up to 40 pounds in the performance of assigned duties.
  • Normal manual dexterity and eye-hand coordination required.
  • Repeated hand-wrist movement required.
  • Ability to use a variety of office equipment and machines as referenced.
  • The Pay Rate for this position starts at $23.20-$24.95 per hour.
  • The nature of the position involves a fast-paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moments notice and the position requires quick response time and flexibility.
