The Permanency and Family Connections Specialist works in conjunction with the clinicians, unit supervisors, and / or direct care staff to achieve both physical (a discharge resource) and emotional permanency. This position is focused on creating and re-establishing relationships to promote more timely and successful discharge planning experiences for youth and families.
Essential Job Responsibilities
- Implement the Family Finding and / or Child-focused Recruitment Model to fidelity with assigned youth.
- Facilitate, scribe, and / or coordinate Permanency Roundtables for referred youth.
- Engage youth, caregivers, relatives, fictive kin and / or other appropriate adults to increase connections and build a lifetime network of support for each youth and their families.
- Support teams in determining the type of permanency intervention needed for youth.
- Educate youth, families, providers, and strategic partners about the positive impact of Family Finding and / or Wendy’s Wonderful Kids permanency services.
- Develop, maintain, and update all required documentation in a timely manner utilizing effective written and oral communication.
- Participation in weekly treatment team meetings to collaborate with the team to best meet the needs of the youth we are serving
- Individual meetings with the youth in our programs to help them “provide their voice” to their permanency plan
- Communicating with identified resources (with releases) and facilitating relationships with the youth
- Develop working knowledge of available services to support caregivers

Special Considerations
Our goal is to maintain a restraint free environment. However, there may be times that staff may be required to physically restrain clients weighing between 50 lbs. and 200 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary.
24.25 Minimum pay rate, $31.50 Maximum pay rate, based on experience.
