Job Summary for Care Coordinator:
- Utilizes a short-term case management model to provide resources, brokering and education for a minimum of three days following the crisis assessment.
- Follows up and links the client with specified services and resources as identified by the therapist or determined beneficial by the Care Coordinator
- Immediately alerts the direct supervisor of any unusual problems, anomalies or potential crisis that arises with clients.
- Complies with agency policies and procedures as well as, funding and regulatory guidelines and standards.
- Ensures that routine paperwork is completed accurately within the required timeframe.
Requirements
Education and Experience Requirements for Care Coordinator:
- Bachelor’s degree in a social services field is required.
- A minimum of one year of experience working with adults in the behavioral health field.
- Good computer skills and ability to adapt to electronic health record changes.
