CADES seeks a Program Coordinator to further our mission of empowering those with special needs to achieve their highest potential.
Position Description: The Program Coordinator oversees and ensures the implementation of all ISPs in an environment that fosters independence and promotes programming that aligns with the CADES mission and vision. Responsibility to ensure compliance with all CADES policies and procedures, state regulations, accrediting measures and insurance standards is key to this role.
Essential Duties and Responsibilities:
Align CADES programs and services with mission, vision and regulatory requirements that position CADES to be a role model in delivering adult day care services to individuals with multiple disabilities and abilities.
Oversight of CSP implementation of all program planning within the guidelines of regulatory standards including assessments, ISPs, monthlies, quarterlies, and support plans.
Develop active programming that meets the interests of the individuals served and aligns with individual ISP/BSPs, Everyday Lives, Community Rule and CADES vision.
Active participation in the Incident Management process, including: entering incidents into EIM, point person for incidents, timely reporting to appropriate entities (APS), ensuring plans of correction for all incidents.
Oversee and manage regulatory monitoring for individuals, including data collection, analysis, response to concerns and implementation of preventative measures.
Act as main liaison for external stakeholders (Supports Coordinator, Family, providers) and internal stakeholders including Nurse, Trainer, Assistant Director and Program Director for all staff & individuals under assigned area of oversight.
Train, monitor & ensure programs are meeting the programmatic, health care, supervision, and dietary needs of all individuals. Ensures safety in both facility and community settings.
Support staff development through performance measures that provide clear and consistent expectations that align with specific duties and responsibilities.
Qualifications:
Bachelors degree with two years experience working directly with persons with disabilities or; Associate's degree or equivalent with four years experience working directly with persons with disabilities.
High school diploma with ten years of experience working directly with persons with disabilities and demonstrated experience supervising Direct Support staff.
Valid drivers license and 3-year adequate driving record.
Consistently presents in a positive, pro-active, open manner. Seeks first to understand; demonstrates ability to meet individuals at their need level. Holds individuals responsible by ensuring a commitment to training, understanding and high expectations.
Ability to create and implement program planning and outcomes measurement.
Knowledge of PA Department of Public Welfare Chapters 2380 and 11 regulations.
Ability to effectively communicate in a verbal and/or written manner.
Ability to handle and maintain confidential information.
Other duties as assigned.
