Summary
As a Case Manager at Children First Community Homes and Services, you will play a vital role in supporting children within the foster care system. This position is essential in ensuring that each child receives comprehensive care, advocacy, and resources to thrive despite their circumstances.
Responsibilities
- Conduct assessments to identify the needs of children in the foster care system.
- Develop and implement individualized service plans tailored to each child's unique situation
- Coordinate with various service providers to ensure comprehensive support for children and families.
- Monitor progress and adjust service plans as necessary to meet evolving needs.
- Advocate for children's rights and well-being within the community and social services framework.
- Maintain accurate documentation of case notes, assessments, and monthly progress reports.
- Facilitate communication between children, families, and service providers to enhance service delivery.
- Provide emotional support and guidance to children and their families throughout the case management process.
- Handle Intake and Placement
- Work on Call and be available to take and coordinate clients to appointments for Medical, Mental and Dental appointments
- Enroll children in school and attend IEP and School Meetings
- Attend staffings, court, and lead Treatment Team Meetings every Thursday from 4 PM to 6 PM.
Must have a flexible weekly schedule, which may include early mornings (7 AM start) or late evenings. Must also be available to work onsite at the group home and travel to all sites to ensure file compliance.
Qualifications
- Bachelor's degree in social work, psychology, or a related field preferred.
- Experience working with children in foster care or similar environments is highly desirable.
- Strong understanding of case management policies and practices.
- Excellent communication skills, both verbal and written, for effective interaction with clients and stakeholders.
- Ability to develop strong relationships with children, families, and community resources.
- Organizational skills to manage multiple cases effectively while maintaining attention to detail.
- Compassionate approach with a commitment to advocating for children's needs.
- Be available on call for emergencies and intakes
