IFCCS is an integrated, intensive, community-based program, grounded in a philosophy of unconditional commitment to partner with clients and families to strengthen and empower relationships by fostering hope, wellness, and resiliency. The most important outcome of the IFCCS program is to promote the safety, permanence and well-being of children and Transitional Aged Youth (TAY) involved with the Child Welfare System. A multi-disciplinary treatment team provides an array of services firmly grounded in the Shared Core Practice Model. Services are intended to expedite access to ICC and IHBS.

Under the direct supervision and leadership of the Program Supervisor the IFCCS Rehab Specialist is a direct service provider who, along with the IFCCS Care Coordinator and Therapist, is a member of the IFCCS team. Participates as member of the Child and Family Team and takes part in developing and implementing the Mental Health Client Treatment Plan (CTP) and the IFCCS Matrix. The IFCCS Rehabilitation Specialist provides direct services and supports to children and their families in a variety of settings. The IFCCS Rehabilitation Specialist is expected to deliver intensive home-based services that are individualized, and strength-based mental health interventions that are designed to ameliorate the mental health condition(s) that interfere with the child’s functioning.


Job Responsibilities:

  • Under the direct supervision of the Program Supervisor and along with the IFCCS Care Coordinator and therapist, takes part in the Engagement process and conducting the strengths-based discovery and assessment with the child and family. Helps them to focus on clarifying those underlying mental health needs that are barriers to the child’s successful functioning.
  • Participates in Child and Family Team meetings.
  • Forms relationships with the child and family and assists them in meeting their goals.
  • Assists the child and family in the home, school and community by providing a variety of supportive Intensive Care Coordination, including urgent service delivery and care planning
  • Provides Intensive Home Based Services (IHBS) that build on individual strengths, concerns, and needs and that are designed to help reduce the mental health concerns/condition that impair the child’s function, with a balanced focus on the family as a unit. Some of these services may include skills-building, mentoring, coaching, behavioral support, and psycho-education.
  • Provides Individual, Family and Collateral therapies as required.
  • Maintains a close and supportive relationship with the child and family, program staff, community supports and any others who may be involved with the child and family.
  • Responds and intervenes as necessary and appropriate.
  • Completes documentation on all services provided within 24 hours of service provision.
  • Provides Program Supervisor with weekly program status on meeting documentation requirements and timelines.
  • Works collaboratively with staff, family members, public and private agencies and community support systems.
  • Participates in all required trainings.
  • Attends and participates in weekly group and individual supervision.
  • Performs other duties as assigned.
  • Complies with all Federal, state and local laws, rules, and regulations.

General

  • Maintains confidentiality in matters of agency operations, personnel, and clients.
  • Maintains positive communications with all agency staff, clients and visitors.
  • Exercises good judgment in performance of duties and responsibilities
  • Supports existing agency policies, principles and mission


Job Qualifications:

  • This position requires a bachelor's degree in Psychology, Sociology, or Human Services Field; Two years of experience in social services may replace the degree.
  • Must possess sensitivity to family culture, values and beliefs, and be able to operate from a family-centered, strengths-based perspective.
  • Clinical knowledge and expertise within scope of practice in group, family and individual therapy.
  • Community resources in Antelope Valley and surrounding areas.
  • Should have experience working as a member of a team.
  • Experience with DMH paperwork preferred.
  • Must possess good written and verbal communication skills, be organized and be willing to work a flexible and varied schedule.
  • Must possess a valid CA driver’s license, be able to drive, and remain insurable.
  • Bilingual helpful
  • Must be willing to transport client/families as approved by supervisor, and as needed and indicated in the IFCCS Matrix.
