The Care Coordinator is responsible for providing patients with appropriate referrals to internal / external resources and agencies, coordinate specialty care for clinic patients, schedule appointments for clinic patients with a provider of a physician, serve as a liaison between the Provider, Registered Nurse (RN), Licensed Vocational Nurse (LVN), and Care Team Supervisor to ensure patients receive appropriate care. Must interact in a positive manner with staff, patients, families, and members of the community.
ESSENTIAL JOB FUNCTIONS :
- Completes routine reports, messages, and forms
- Communicates in written and verbal form, using correct grammar
- Submits all correspondence within the requested time frame
- Relays all messages as instructed
- Organizes and files record according to established clinic procedures
- Files records in chronological and alphabetical order
- Effectively utilizes office machines to include a computer, fax, and photocopier
- Collects and distributes mail in a timely manner, and in accordance with established clinic procedures
- Participates in huddles
- Schedules appointments
- Takes messages for the Care Team
- Follows all policies and procedures
- Sends authorizations and referrals, completes and documents follow-up on authorizations and referrals to outside agencies, as determined by medical staff to ensure continuity of patient care

Work Environment
The noise level of the work environment is usually moderate. Workspace is in an indoor office setting, shared workspace, air-conditioned in the summer and central heat during the winter. Occasionally required to be outdoors for travel, and can be exposed to variable weather conditions.
