Description

SUMMARY
This position serves as a key liaison between patients, families, healthcare providers, and community resources, ensuring seamless access to care, education, and advocacy. The role is instrumental in improving health outcomes for children with complex medical needs by implementing strategic patient engagement initiatives, identifying systemic barriers, and driving program enhancements.

The Community Health Liaison provides expert-level education on healthcare systems, ensuring patients and families maximize available resources. Additionally, this role fosters partnerships with healthcare providers and community organizations, promoting the Children’s Clinics’ multidisciplinary care model to enhance referrals and access to specialized services.

ESSENTIAL FUNCTIONS

  • Serves as a community health advocate, leveraging expertise in complex pediatric care coordination to enhance patient and family engagement.
  • Executes strategic outreach efforts to hospitals, primary care offices, and community organizations, ensuring early identification and referral of children who would benefit from Children’s Clinics services.
  • Designs and facilitates educational programs that equip families with the knowledge and tools to navigate healthcare systems such as AHCCCS CRS, the Division of Developmental Disabilities, and other critical care networks.
  • Conducts comprehensive home and community-based assessments, identifying social determinants of health barriers and implementing tailored solutions to improve patient access and adherence to care plans.
  • Develops and implements process improvements to streamline patient navigation, reducing gaps in care and enhancing interdisciplinary coordination within the Children’s Clinics.
  • Represents Children’s Clinics in high-level community health initiatives, collaborating with external stakeholders to advocate for improved services and policy changes benefiting children with medical complexities.

  • Expertise in healthcare navigation, advocacy, and the unique needs of pediatric patients with complex medical conditions.
  • Proven ability to execute strategic outreach initiatives that improve patient engagement and access to care.
  • Proficiency in problem-solving, critical thinking, and process improvement, with an emphasis on health equity and social determinants of health.
  • Exceptional communication skills, including the ability to engage diverse audiences—patients, families, healthcare providers, and policymakers—through both verbal and written communication.
  • Proficiency in data-driven decision-making, leveraging insights to optimize patient education and community outreach strategies.
  • Skilled in using electronic health records (EHR), data analysis tools, and digital platforms to MINIMUM QUALIFICATIONS.

Qualifications


Education:

Master’s degree in the field of social work, psychology, or related subject area

Community Health Worker certification or relevant experience and qualifications to be certified in the state of Arizona as a community health worker.

Experience: 

Relevant experience to qualify as a community health worker. Experience with both pediatric and adult populations preferred.

Licenses and Certificates:

Community Health Worker certification

FINGERPRINT CLEARANCE

Employees are required to maintain a current Arizona Department of Public Safety Level 1 Fingerprint Clearance Card. Evidence of a current and valid fingerprint clearance card must be received to begin and maintain employment.

While performing the duties of this job, the employee is frequently exposed to toxic or caustic chemicals.  The employee is occasionally exposed to fumes or airborne particles, risk of electrical shock, and risk of radiation.  While performing the duties of this job, the employee may potentially be exposed to infectious organisms during routine and emergency situations. The noise level in the work environment is usually moderate.

Category 1: Performs tasks that involve exposure to blood, body fluids or tissue.  Use of appropriate protective measures should be required.
