What are the responsibilities and job description for the Care Coordinator – Blended Case Management (BCM) position at Children's Crisis Treatment Center?
CCTC is seeking a Care Coordinator for the Blended Case Management Program to support and follow through with treatment team recommendations in a fast-paced environment and meet the needs of the children ages 2.5-21 and their families presenting with crisis and behavioral health issues in a trauma-informed manner.
Responsibilities include;
Requirements
Bachelor’s degree in Social Work, Social Science, Nursing or Bachelor of Arts Degree from an accredited educational institution.
Minimum of one (1) year case management or direct care mental health experience with children and families.
Minimum of one (1) year case management or counseling experience with children and families.
Bilingual (English/Spanish) applicants strongly encouraged to apply.
COVID-19 vaccination required.
Responsibilities include;
- Developing and implementing service coordination plan in conjunction with treatment team
- Identifying, linking, and coordinating of services for children and families with CBH, other service providers, schools, etc.
- Monitoring effectiveness of services and reassessing service needs if necessary
- Maintaining timely and accurate electronic chart records
- Ensuring the completion of necessary documentation and outcome measures, and signatures obtained on relevant treatment documents
- Participating in internal as well as external trainings ensuring completion of CCTC mandatory trainings
