About the role!

The Coordinator of School-Based Transitions- EBC is responsible for supporting students' transition to EBC and from EBC. The Coordinator develops individualized plans for students' first 30 & 60 days within the EBC program. Additionally, the Coordinator is responsible for designing and supporting the transition for students graduating from EBC. This includes connecting with School Districts, parents, and the site interdisciplinary team. This position supports both EBC campuses and is on-site daily. This position currently reports to the Director of Operations.

As Coordinator of School-Based Transitions, you will:

  • Develop, design, and collaboratively implement instructional, behavioral, and therapeutic plans for students newly enrolled in EBC.
  • Assess data to determine the effectiveness of students' plans.
  • Develop a strong knowledge of programs both public and NPS.
  • Work in conjunction with site and district IEP teams.
  • Collaborate with families and on-site multidisciplinary teams.
  • Implement the use of the phasing plan for all students within EBC.
  • Develop and support the implementation of plans to support transitions to placements outside of EBC.

  • Flexibility in scheduling appointments and punctuality in meeting all commitments.
  • Knowledge of computer applications relevant to school planning and scheduling (e.g. Outlook and Zoom).

About your background (education/experience):

  • Must possess a degree in a related field (education, psychology, business)
  • Master’s Degree is preferred.
  • Candidate must have 5 years of work experience within an NPS.
  • Public School experience is highly preferred.
  • Ability to work in a fast-paced, dynamic environment.
  • High level of understanding and interpreting of IEP reports.
  • High level of organizational skills.
  • Strong written and oral communication skills.

Working Conditions

Physical Requirements:

  • The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.
  • Ability to undergo training and implementation of Crisis Response Training.
  • Ability to stand and sit for extended periods.

Special Job Requirements:

  • It will be necessary for the Coordinator of School-Based Transitions to occasionally attend after-school or agency-wide events that take place after regular school hours.
  • The Coordinator of School-Based Transitions is expected to attend such events as they are scheduled.
  • 220-day calendar to be determined by the employee and supervisor. This is an on-site position.

  • Extensive interpersonal interaction with peers and outside contacts.

Compensation:

Range: $90,000-$115,000 per year

Pay Type: salary

  • The starting base salary for this position is as shown above. The actual base salary is dependent upon a variety of job-related factors such as professional background, training, work experience, location, business needs, and market demand. This pay range is subject to change and may be modified in the future.
