This position works closely with a multidisciplinary team that focuses on the goal to achieve optimal asthma control and decrease the risk of adverse events. In addition, the asthma education coordinator monitors key metrics and analyzes information to ensure operational quality and efficiency targets are being met.

Responsibilities
- Ensure Home Management Plan of Care (HMPC) is correctly completed with information specific to the patient
- Review discharge instructions or AVS with patients and families
- Verify age appropriateness of medications and dosages per child’s age and weight
- Ensure patient is going home with necessary asthma equipment for home and/or school
- Review Asthma Action Plan if applicable
- Ensure Pulmonary patients have an appointment or specialist has been notified
- Review pertinent patient data (laboratory tests, diagnoses, home skills needed, medical history, diagnostic procedural results, etc.) to aid in developing education plan.
- Answer any asthma education related questions families have prior to discharge
- Collaborate with patient, family and interdisciplinary care team to establish goals for self-management skills training and education.
- Assess the educational needs of the patient and caregivers and form an education plan for safe care.
- Provide initial and continuing education and skills training for children and caregivers on the care that will be required upon discharge from the hospital and across the care continuum.
- Document patient/caregivers learning needs assessment, goals, management plans, and understanding of education provided. Apply health literacy principles at every education encounter with a patient family.
- Educate and mentor other disciplines around the impact of health literacy on patient care and outcomes.
- Develop content and readily make available on a variety of media or technology platforms to deliver health education.
- Assist with the download and understanding of the Children's Health Asthma Buddy App.
- Collaborate with care team to ensure patient and caregivers have all needed resources when discharged and at touch points along the care continuum.
- Demonstrate the knowledge and skills necessary to provide care appropriate to patients ranging in age from 0 to 18 years.
- Demonstrate knowledge of the principles of growth and development over the life span.
- Possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs.
- Provide the care needed as described in the unit's policies and procedures.
- Promote and participate in research, evidence-based practice, and quality improvement initiatives in the clinical setting.
- Comply with state, federal and other regulations.
- Maintain patient safety, privacy and confidentiality
- At least 3 years licensed clinical professional (ex. RN, RRT) required
- At least 3 years Pediatric clinical experience preferred
- At least 3 years Asthma education experience required
- Four-year Bachelor's degree or equivalent experience BSN required for RN required
- Graduate or professional work or advanced degree; or equivalent experience preferred
- Asthma Educator Certification (AE-C) 1 year required
- Current license to practice professional nursing in the state of Texas or respiratory care in the state of Texas upon hire required
- If RCP required, will need RRT recognized by the National Board of Respiratory Care (NBRC) upon hire required
- Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements upon hire required
- PEARS/PACES 180 Days upon hire required
