Summary
The Coordinator independently performs the functions of a registered nurse in direct patient care. Encompasses specialized nursing care focused on palliative care patient population. Responsible for working with the multi-disciplinary team to coordinate, oversee, and monitor medical and psychosocial activities related to palliative and hospice care, and provide a comprehensive plan of care to meet the patient/family needs. Utilizes the nursing process in the delivery of developmentally appropriate care. Utilizing advanced nursing skills and knowledge, the coordinator is responsible and accountable for coordinating care for the assigned patient population.

Utilizing advanced nursing skills and knowledge, the coordinator is responsible and accountable for coordinating care for the assigned patient population.
Responsibilities
- Evaluate palliative and hospice recipients and potential recipients by obtaining personal and medical history, reviewing diagnostic and laboratory test results, and scheduling medical/psychosocial consultations as warranted.
- Assist medical providers with medical treatment and provide plan of care information to patients and/or family members.
- Sick call & symptom management of palliative and hospice patients, requiring response within one hour of a call during business hours. This includes collaboration with outside multidisciplinary teams to provide plan of care information to patients and/or family members.
- Hospice liaison responsible for introducing the idea of hospice alongside medical providers and coordinating care between inpatient teams and outpatient hospice teams for patients with complex medical needs.
- Building rapport and having sensitive and potentially controversial conversations with families outside the hospital.
- Participation in care conference multidisciplinary discussions for palliative and hospice inpatients.
- Collaborating with interdisciplinary team to manage symptoms and optimize hospital throughput including navigating complicated discharges.
- Develop and provide education regarding the palliative and hospice evaluation process and care. Also provide ongoing education that begins during the initial palliative admission and continues throughout the patients palliative and hospice related care at Children's.
- Integral role in the development, implementation, and process improvement of end-of-life collaboration including development of guidelines and educating inpatient and supporting staff.
- Document accurately & thoroughly all patient encounters in electronic health record. Ensure that Patient Health Information (PHI) is protected and secured at all times. Adhere to all infection prevention initiatives (i.e., hand hygiene, proper disposal of waste, etc.).
- At least 3 years Pediatric nursing experience required
- Four-year Bachelor's degree or equivalent experience Bachelors of Science in Nursing required
- Current license to practice professional nursing in the State of Texas Upon Hire required
- Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements required
- PACES 180 Days required
- CHPN (Certified Hospice & Palliative Nurse) offered through the Hospice & Palliative Nurses Association required within 24 months of employment. required
