Summary :
This position functions under the direction and supervision of the Registered Nurse in providing care and implementing delegated activities of Hemodialysis treatment and related renal care to patients.
Responsibilities :
- Responsible for the accurate documentation of information related to the patient treatment. Assist RN with pre- and post-dialysis patient assessments and their documentation. Monitor and document dialysis treatment parameters on dialysis flow sheets. Report any significant information or change in patient condition to the RN. Assist with all emergency operational procedures. Initiate basic CPR measures in the event of cardiac and / or pulmonary arrest, and respond to emergency situations related to dialysis treatment.
- Patient care functions : Receive patient into treatment area. Weigh patient and take blood pressure, both pre- and post-dialysis, and document on dialysis flow sheets. Perform and record patient observation (vital signs and all other parameters of ESRD Patients - obtains blood sample and culture specimens and prepare for shipment). Review dialysis orders, note general physical and mental condition of patient through observation / interview, provide data to determine compliance with dietary or medical regimen, and document observations appropriately. Provide chronic hemodialysis therapy, recommend a dialysis plan for ultrafiltration to RN, initiate dialysis via approved accesses, observe all safety measures, monitor patient vital signs applicable to critically ill patients, administer fluid therapy or other appropriate treatment as directed, document activity appropriately. Collect patient data, monitor and document status of vascular accesses, review lab work, patient observation and seek guidance when levels are out of normal ranges for ESRD patients. Perform the following only under the direction of an RN : a) Instruct patients in the care of fistulas and open wounds, b) review physician orders and reinforce treatment care with patient.
- Equipment care : set up and monitor dialysis machines and systems. Observe dialysis machines and RO water systems for correct functioning. Respond to visual and audible alarms and make appropriate adjustments pre-, during and post-dialysis treatment. Dismantle, clean and disinfect dialysis machines and equipment according to CMC procedures. Understand all duties related to dialyzer : dialysis cleaning and sterilization and equipment using chemicals, rinses, priming the dialyzers, attachment and installation of all tubing, positioning of dialyzer, preparation of dialysate delivery system for dialysis, dialyzer connection and all pumps, alarm checks, monitor settings and dialyzer tests.
- Water treatment & Dialysate preparation : Open the dialysis unit as assigned in the absence of the equipment tech, prepare dialysate as assigned in the absence of the equipment tech. Document water treatment & dialysate preparation data on the appropriate logs.
- Quality Assurance : Complete flow sheets to ensure appropriate billing. Report inventories and supply shortage to Charge Nurse, Renal Equipment Technician and / or Renal Technical Manager. Comply with Children's Health Clinical Practice Policies and in accordance with state and federal regulations for patient care services, injury prevention and safety training. Participate in hospital and government quality assurance programs.
