POSITION SUMMARY :

Assists in providing a range of case management services to a diverse Akin client population including participation in assessment of individual client needs. Has access to protected healthcare information (PHI) or confidential identifiable information (CII), both paper and electronic, as necessary, to perform related job duties and responsibilities.

ESSENTIAL FUNCTIONS :

  • Implements an already established treatment / service plan for assigned consumers by providing a variety of educational, and recreational services
  • Meets with consumers at an established location, primarily within a school system
  • Provides case management assistance
  • Maintains electronic databases, consumer records and ensures confidentiality
  • Performs routine administrative duties
  • Establishes and maintains collaborative relationships with appropriate community partners
  • Advocates for the values of a diverse population within Akin and the Community
  • Performs other duties as assigned by Supervisor

Additional Duties :

  • May perform one or more of the following duties, as assigned by supervisor :
  • Direct Service Volunteer recruitment, coordination,
  • Foster care / post-adoption recruitment, placement,
  • Consumer resource development and coordination and / or community outreach
  • Case management assistance for a specialized program
  • Provide mentorship to new employees and interns
  • KNOWLEDGE, SKILLS, AND ABILITIES :

  • Must possess strong human relations skills to communicate and work effectively within a culturally diverse work environment and community
  • Proficient in MS Office
  • WORKING ENVIRONMENT :

    This job is performed in a professional office environment. The following list is representative of the work environment employees encounter while performing the essential functions of this job :

  • WORKING REMOTELY :

    Working remotely is a benefit of Akin’s flexible workplace, but remote work cannot be offered for all positions. Some positions require daily presence in the office. Remote work arrangements may be adjusted or rescinded based upon business needs.

    PHYSICAL DEMANDS :

  • While performing the essential duties of this job, the employee is regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer and telephone keyboard; occasionally to reach, stoop and kneel
  • Specific vision abilities required by this job include close vision requirements due to computer work
  • Regular, predictable attendance is required

    MINIMUM QUALIFICATIONS :

  • Two years’ experience in direct service to children, youth, and families or a combination of education and experience that demonstrates the knowledge, skills and abilities required
  • Experience and understanding of child, adolescent, and family development and of various treatment principles and interventions
  • Agency Affiliated Counselor credential obtained after hire
  • A valid driver’s license is required
  • CPR, First Aid Certifications (within first 30 days)
  • A one-step Mantoux TB skin test prior to start of employment. For those who have previously tested positive, documented proof of treatment from a doctor or a negative chest x-ray must be provided
  • Must be available to be on call 24 / 7 (21 and older
  • Successful completion of a drivers check if position requires driving on a regular basis (at least three times per week) or drives clients. Must not have more than two driving incidents within the last three years
  • PREFERRED QUALIFICATIONS :

  • BA degree in related field
  • SALARY RANGE AND BENEFITS :

    The salary range for this position is $20.42-$27.55. The starting salary range for this position is $20.42-$22.20 DOE for external candidates.

    Employees (and their families) are covered by medical, dental, vision, and basic life insurance. Employees are able to enroll in our company’s 401k plan. Employees will also receive 10 days of vacation leave each year, three personal days each year, and will accrue one sick day per month (pro-rated for part-time employees). Employees will also enjoy 14 paid holidays throughout the calendar year. We also offer EAP and mental health benefits.
