POSITION SUMMARY:
We are establishing a new, Critical Time Transition (CTTP) Program, in Broome County, designed to support youth during pivotal transitions in their lives, such as moving from institutional care to community-based living. The program provides short-term, intensive case management to foster stability, independence, and continuity of care. CTTP focuses on building sustainable support networks by linking participants to community resources, mental health services, housing, and employment opportunities. Through collaboration, the program ensures youth are equipped to navigate challenges and reduce the risk of setbacks during critical periods.
The CTI Case Manager provides trauma-informed, individualized case management services to children, youth, and families enrolled in the CTTP program. Under the supervision of the Program Director, they develop and implement success plans to provide immediate support during crises, stabilize circumstances, and connect clients with community resources. They collaborate with the CTTP team, families, schools, courts, and other stakeholders to ensure continuity of care and foster long-term stability. The Case Manager also guides the Educational/Vocational Specialist and maintains consistent contact with clients through in-person, virtual, or phone interactions.
RESPONSIBILITIES:
- Work closely with at-risk populations, including individuals transitioning from Comprehensive Psychiatric Emergency Programs (CPEP), to promote healthy and safe transitions to stable home environments.
- Assertively engage youth and families during critical transition points, such as hospital discharge or mental health treatment, to reduce the risk of out-of-home placement.
- Assist with success planning, goal setting, service coordination, and advocacy to address client needs effectively.
- Conduct comprehensive assessments to identify immediate needs, strengths, and potential challenges during transitions, and develop personalized crisis intervention and stabilization plans.
- Coordinate services and referrals, connecting youth with community providers and support systems for long-term care.
- Monitor client progress, adjust interventions as needed, maintain accurate case notes in CONNECTIONS, and comply with agency policies, procedures, and training requirements.
EDUCATION REQUIREMENTS:
- Bachelors or Master's in Social Work or related field required
EXPEREINCE REQUIREMENTS:
- Two or more years experience in child, adolescent, and family experiences with transitional or crisis circumstances preferred
- Knowledge of evidence-based trauma informed practices, proactive assessments, and screenings preferred
- Valid Drivers License required
