What are the responsibilities and job description for the County Lead - Community Based Services position at Children's Home?
WHO WE ARE: The Children’s Home of Wyoming Conference partners with children, families and communities throughout New York State to inspire hope, develop skills and cultivate healthy relationships for positive futures.
Position Summary: The County Lead supervises Community Based Providers in Sullivan County. The County Lead is responsible to ensure all program guidelines are followed and services are properly documented in the electronic health record.
Responsibilities:
- Manage referrals from a variety of resources
- Maintain collaboration with clients, families, staff, programs, and community contacts
- Provide regular supervision to staff
- Facilitate staff meetings
- correspond with leadership with the program and agency
- Work collaboratively with other members of the treatment team
- Actively work to maintain program growth and sustainability
Education:
- A Bachelor’s Degree is required
Experience:
- 2 years of related experience working with children and youth with mental health concerns, addiction, or in foster care settings is required
- Driver's License required
