What you will do in your role:
The Foster Care Case Planner uses a diverse and strength-based approach for the direct interaction with the child, the foster parents, the child's birth family, and the community. The Case Planner acts as a liason with various private, as well as, public service providers and resource agencies to coordinate all case management needs of the youth and family.
- Develops and sustains ongoing working relationships with the child, foster family, and birth family.
- Prepares placement and discharge summaries.
- Writes treatment and crisis management plans
Qualifications:
- Bachelor's Degree in Human Services or related field from an accredited college or university preferred.
- Minimum Associate's Degree in human services or related field from an accredited college or university required.
- 2 years experience in child welfare and/or foster care required.
- Must have valid Driver's License and ability to maintain insurance throughout employment.

Pay: $20.00 - $22.00 per hour
