What are the responsibilities and job description for the Foster Care Case Planner position at Children's Home?
The Children's Home is adding a Foster Care Case Planner to our team in Sullivan County!
The Foster Care Case Planner uses a diverse and strength-based approach for the direct interaction with the child, the Foster Parents, the child's birth family, and the community. The Case Planner acts as a liaison with various private as well as public service providers and resource agencies to coordinate all case management needs of the youth and family.
Job Duties:
- Develops and sustains ongoing working relationships with the child, foster family, and birth family
- Prepares placement and discharge summaries
- Writes treatment and crisis management plans
Qualifications
- Bachelor's degree in related field
- 2 years' experience in child welfare and/or foster care
