Are you ready to make a meaningful impact in the lives of young people? As a Psychiatric Nurse Practitioner in our Sullivan County Youth Assertive Community Treatment (ACT) program, you’ll be a vital member of an interdisciplinary team dedicated to helping children ages 10-21 avoid hospitalizations and out-of-home placements. By delivering intensive, highly coordinated, and personalized services, you’ll play a crucial role in addressing the significant functional challenges and severe symptoms experienced by youth facing mental health issues or serious emotional disturbances.
What You Can Expect as a Youth ACT Psychiatric Nurse Practitioner with the Children’s Home:
- Collaborative Environment: Enjoy strong support and open communication within a team focused on providing intensive services to 28 youth and their families.
- Specialized Training: Receive in-depth training in the Youth ACT model, along with essential skills in person-centered planning, motivational interviewing, trauma-informed care, social determinants of health, cultural competence, implicit bias, and other best practices.
Your Key Responsibilities:
- Conduct psychiatric and medical assessments
- Prescribe and manage medications
- Collaborate closely with the Youth ACT team on medication and service coordination
- Partner with Youth ACT team providing community-based services
- Provide education and training to the team
- Participate in daily team meetings and treatment planning
- Communicate with external health providers as needed
- Offer psychoeducation to clients and families
- Meet with youth in their home or community as needed
Your Education and Qualifications:
- Master of Science in Mental Health Psychiatric Nursing from an accredited institution required
- State of New York certification as a Nurse Practitioner in Psychiatry with prescriptive privileges required
- DEA License required
- Experience in prescribing psychotropic medications to children and adolescents across multiple diagnoses required
- Five years of experience as a psychiatric clinic nurse specialist or psychiatric nurse practitioner preferred
