A Brief Overview

This role oversees the case management operations and provides supervision for the Violence Intervention Program (VIP).

What You Will Do

  • CLINICAL SUPERVISION
  • Oversee the day to day case management operations of the Violence Intervention Program including referrals, eligibility, intake reviews, documentation, and case goals.
  • Oversee and collaborate in care management including assigning cases and transfers
  • Oversee and as needed, perform psychosocial assessments, therapy, and group interventions with clients and/or caregivers
  • Share information regarding patient/caregiver situations with stakeholders including with staff from community agencies and programs
  • Ensure compliance and incorporate standards and regulatory requirements into care
  • Communicate program status to stakeholders and escalate and resolve potential issues
  • Hire, train, supervise and develop staff including case managers, specialists and interns
  • PERFORMANCE MANAGEMENT
  • Review case plans regularly and discuss barriers and solutions to meet goals
  • Develop and utilize staffing strategies to meet client needs and the client care populations
  • Evaluate of staffing models to meet care needs and ensure work/case load distribution
  • PROGRAMMATIC LEADERSHIP
  • Manage program development including policies, procedures, billing strategies, resources and materials
  • Lead initiatives to promote program growth, improvement and sustainability
  • Establish partnerships with and present programs to internal and external audiences
  • Develop and maintain relationships with patients, families, staff, leaders and physicians
  • Promote and ensure sensitivity to cultural diversity through work and with teams.
  • Identify barriers for families in accessing community services and advocate on their behalf.
  • Identify resources to support client and families recovery goals
  • Other duties as needed.

Education Qualifications

  • Master's Degree required
  • Master's Degree Social Work preferred

Experience Qualifications

  • At least five (5) years social work or healthcare related experience required
  • At least one (1) year managerial or supervisory experience required (3 years preferred)
  • Working with children and diverse populations or providing culturally sensitive counseling services to urban minority populations preferred

Licenses and Certifications

  • Driver's License - Department of Motor Vehicle - upon hire - Required
  • Licensed Clinical Social Worker (LCSW) (Pennsylvania) - Pennsylvania State Licensing Board - - Preferred

  • Ability to operate a company vehicle/be insured on a CHOP policy
  • Basic proficiency with data management systems.
  • Intermediate knowledge of trauma-informed care.

  • Ability to maintain confidentiality and professionalism
