- GENERAL SUMMARY
- The Diabetes Care and Education Specialist functions in an advanced clinical role to provide and coordinate diabetes self-management training within the pediatric diabetes program and services. Reports to designated Director/Manager and works in close cooperation with and under the guidance of the program Medical Director.
- ESSENTIAL DUTIES AND RESPONSIBILITIES
- Develops and participates in formal and informal patient, community and staff education for diabetes self-management training.
- Provides age, cultural and developmentally appropriate psychosocial training and services within a pediatric diabetes center.
- Identifies educational and/or learning needs in collaboration with the management team.
- Functions as a professional resource for staff and the organization to enhance the diabetes program.
- Performs other duties and specialized tasks as assigned.
- LICENSES AND/OR CERTIFICATIONS
- Registered Nurse or Registered Dietitian.
- Current certification as a Diabetes Care and Education Specialist (CDCES) or must attain certification within two (2) years of hire.
- CPR Training required and/or must be obtained within 45 days of hire.
- MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
- Professional knowledge of theory and practice as it relates to diabetes self-management training and diabetes program coordination.
- Pediatric diabetes experience preferred.
- Basic understanding and ability to utilize Microsoft Office and outlook applications.
- Ability to travel to provide diabetes program services within the health system and community to meet the population identified needs.
- WORKING CONDITIONS
- Clinical patient care environment with minimal exposure to hazards such as excess noise, dust, or extremes in temperature. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, body fluids and other conditions common to the healthcare environment.
