What are the responsibilities and job description for the Case Manager - ECM position at Children's Legacy Center?
The Enhanced Care Management (ECM) Case Manager is responsible for providing comprehensive resources, advocacy, and direct support to a diverse population in need of assistance with substance use, medical or behavioral health concerns, food insecurity, and other social determinants of health. This position requires strong collaboration with internal programs and external community-based organizations to ensure clients receive the full spectrum of resources and services available.
The ECM Case Manager must have expertise in harm reduction strategies, substance use treatment, and community resources. This role is integral to the Cal Aim payment reform initiative and requires proficiency with Electronic Health Records (EHR) systems, client enrollment, care plan development, service billing, and completing required documentation in a timely manner.
In addition to technical skills, the ECM Case Manager must demonstrate empathy, effective communication, time management skills and the ability to build and maintain positive, trusting relationships with clients. A thorough understanding of relevant processes and the ability to navigate complex client needs will be essential for success in this role.
This is an in-person position, with working hours from Monday to Friday, 8:30 a.m. – 5:00 p.m.
Duties and Responsibilities:
- Complete thorough Client Needs Assessments following initial Client Services intake
- Perform ongoing assessments, safety planning, and creation of a Care Management plan
- Provide education and support as needed, including education around Domestic Violence, Sexual Assault, Parenting, Substance Use, etc.
- Actively promote client empowerment and self-advocacy
- Maintain thorough and accurate case notes and documentation to track client progress
- Complete initial engagement documentation and meet monthly client billing requirements
- Ensure all informed consents are signed and protocols for information sharing are adhered to
- Provide intensive case management including referrals to services, and regular check in with clients and assisting with transportation to ensure attendance for important appointments.
- Present case information at case staffing meetings to ensure coordinated care
- Participate in shelter case staffing, as applicable.
- Participate in shelter case staffing and attend weekly leadership meetings to assess client service and operational goals
- Other duties as assigned
Knowledge, Skills and Abilities:
- Deep understanding of trauma-informed care principles and best practices in supporting survivors of violence
- Proven ability to manage complex cases and navigate multiple service systems effectively. Demonstrating leadership and initiative
- Strong computer skills and proficiency in case documentation; adept in Microsoft Word, Outlook and other relevant software
- Excellent judgment and problem-solving skills to navigate complicated situations while collaborating with a diverse team of professionals
- Experience working with culturally diverse populations and a desire to implement best practice in addressing their needs to bring about equitable outcomes. Respect for other’s experiences, opinions, language, culture and knowledge
- Comprehensive knowledge of community resources and demonstrated ability to ensure effective service coordination
- Valid California driver’s license and access to reliable transportation
- Strong communication skills, both within a team setting and with clients
- Time management skills are essential
Education and Experience:
- BA or BS degree in in social work, human services, psychology, or related field preferred; or a minimum of three year’ experience working with survivors of domestic violence, sexual assault, and/or human trafficking
- Basic knowledge and ability to work in computer programs (i.e. Word, Excel, etc.)
- Utilizes Trauma Informed Care and understanding of ACES when working with clients.
- Satisfactory completion of 66-hour domestic violence and sexual assault intervention training (post-hiring requirement) and ongoing completion of continuing education hours to maintain state certification as a domestic violence and sexual assault advocate/counselor
- Ability to pass a comprehensive background investigation including fingerprinting
Physical Requirements:
- Ability to navigate the shelter facility and assist clients in various areas, which may include walking up and down stairs or moving between offices
- Capability to sit for extended periods while conducting interviews or paperwork, as well as standing for various tasks such as providing support or assistance
- Ability to lift and carry items up to a certain weight (e.g., up to 25 pounds), which may include files, supplies, or assistance for clients if needed
Rate of Pay:
Full Time, Nonexempt
$22.00 - $30.00 an hour DOE

We currently achieve this goal by offering forensic interviewing services as a part of our Children’s Advocacy Center. These interviews reduce trauma and help our partner agencies expedite justice for children, youth, and their families. We are now able to build upon this tested and stable foundation and are building out our regional pilot model for exploited children and youth.
