Overview
The primary responsibility of the School Based Social Worker is to mobilize family, school and community resources to enable students to learn as effectively as possible through prevention, education outreach, clinical activities, and related services to eligible students. As a key member of the care team or mental health team, the social worker will partner with school administrators, teachers, counselors, families and community partners to develop and implement plans that provide specialized support for selected students throughout the course of the school year.

Responsibilities
Screening, Assessment and Case Management :
- Identify, in collaboration with other school personnel, at-risk students appropriate for social work intervention
- Provide high quality direct clinical services including crisis intervention, assessment, support and case management to at risk students and their families
- Facilitate group sessions for identified students
- Act as the primary point of contact for any concerns of abuse
- Coordinate initial and ongoing intervention strategies with school personnel and other team members
- Ensure the safety and well being of students at risk by helping families navigate systems and address barriers to accessing needed services including housing, food security, mental health, physical health, drug and alcohol treatment
- Complete district, or school specific, training on curriculum to support students and lead or co-lead sessions with identified students
- Intervene with students with significant attendance issues
- Provide case management services for students on home bound services, including home visits, with the goal of reintegration into the classroom setting
- Provide case management services to students on a 504 plan with a primary mental health disability
- Provide sobriety support to identified students

Starting Pay
Our pay ranges are market competitive. The pay range for this job begins at $28.25 / hr, but your salaried offer will be determined based upon your education and experience.
Remote Work / Work from Home
This position is not eligible to work remotely, which means that the person hired will be required to work onsite at one of our Children’s Mercy locations and may not work from home.
