The Department of Pain, Palliative Care, and Integrative Medicine at Children’s Minnesota is seeking a full-time Social Worker, LICSW. The social worker will join a robust interdisciplinary team that includes 5 physicians, 7 APRNs, 2 social workers, 3 psychologists, physical therapists, child life specialist, spiritual care provider, and support/administrative staff. POSITION SUMMARY This position requires a compassionate, resilient individual capable of providing high-quality psychosocial care in emotionally challenging circumstances. The social worker will be an integral part of our interdisciplinary team, working to enhance the quality of life for children with life-threatening conditions and their families. The position involves working across multiple settings including inpatient care and the outpatient clinic in Minneapolis, telehealth, and home care, ensuring comprehensive support for patients wherever they are. KEY RESPONSIBILITIES - Psychosocial Assessment and Intervention: Conduct comprehensive psychosocial assessments of patients and families. Develop and implement intervention plans to address identified needs. Provide counseling and support to patients and families throughout the illness trajectory - Communication and Care Coordination: Facilitate communication between patients, families, and the healthcare team. Assist in coordinating care across various settings (hospital, home, community). Participate in interdisciplinary team meetings and contribute to care planning - Decision-Making Support: Help families navigate complex medical decision-making processes. Advocate for patient and family preferences in care planning. Assist in the completion of advance directives when appropriate - Grief and Bereavement Support: Provide anticipatory grief counseling to patients and families. Offer bereavement support to families, including siblings, for at least 13 months following a patient's death - Community Resource Navigation: Connect families with community resources (e.g., financial assistance, respite care, support groups). Collaborate with schools, faith communities, and other organizations to support patients and families - Education and Advocacy: Educate patients, families, and staff about the psychosocial aspects of pediatric palliative care. Advocate for patient and family needs within the healthcare system and the broader community. - Cultural Competence: Provide culturally sensitive care, respecting diverse beliefs and practices. Assist in resolving cultural conflicts that may arise in care planning.

Qualifications:

LICENSE/CERTIFICATION/EDUCATION REQUIREMENTS - Master's degree from an accredited social work program. - Minnesota state licensed LICSW DESIRED SKILLS/EXPERIENCE: - Experience in pediatric healthcare or palliative care - Comfortable working in home-care settings and ability to self-transport to patient/family homes (if using car, required to have car-insurance and driver’s license) - Strong communication and interpersonal skills - Knowledge of child development and family systems - Experience in grief counseling and crisis intervention - Cultural competence and ability to work with diverse populations - Comfortable working with seriously ill children and their families Physical Demands Please click here to view the Physical Demands All job offers are contingent upon successful completion of an occupational health assessment, drug screen, background investigation, and compliance with the U.S. Government Form I-9, Employment Eligibility Verification.
