Job Description - Sr Manager of Care Management (240003C6)This position will require employees to be in the office or in the field (D.C) four (4) days a week.The Associate Director will assist the Director of Care Management to help assess and evaluate the care management services rendered, staffing levels, competency and educational needs of the care management staff and effectively coordinates and controls resources. The Associate Director oversees the Supervisory Care Management Staff, supporting the needs of the department and promoting delivery of the care management model in partnership with the Director, and drives performance goals outlined in the annual work plan. This position has a broad knowledge base related to the needs of the Medicaid population and is an integral member of the Care Management Leadership team. The Associate Director supports the operations of the Care Management Department through monitoring of staff's adherence to quality and productivity standards; assisting with responding to enrollee's needs; and providing clinical supervision and leadership for compliance and performance improvement.Qualifications Minimum EducationMaster's Degree Nursing, Social Work, Health Services Field (Required)Minimum Work Experience5 years Successful operational management experience, oversight of programs, and supervision of management level direct reports. (Required)7 years Experience in care / case management. (Preferred)Required Skills / KnowledgeSkills in problem-solving and collaboration with others.Demonstrated experience in implementing initiatives or projects required.Demonstrated leadership skills required.Strong problem solving and strategy execution abilities. Clinical Social Worker Licensed in the District of Columbia Upon Hire (Required) OrRegistered Nurse Licensed in the District of Columbia Upon Hire (Required) Certified Case Manager 1 Year (Preferred)Job FunctionsEssential Job DutiesSupports the Director of Care Management in administering the daily operations of the CM department.Assists in developing strategy for change and implementation.Supervises 5-9 staff, including supervisory care managers, DDS managers, discharge planners, and disease managers.Builds, develops, and sustains high performing people and teams.Ensures teams are focused, competent, and productive in executing policy and procedure.Responsible to assist the supervisory CMs in addressing Enrollee needs, resolve enrollee issues, and build the capacity of others to resolve complaints and grievances.Assists with development and revision of CM policies and procedures.Promotes operationalizing policy and procedures through people, specifically coordinating with Director, CM to : Deploy training, skill-building, and tools to support the defined processes.Pace the roll-out properly to balance speed with the avoidance of rework.Enlist Supervisory Care Managers in accessing the reports / data available to them to identify emerging issues and trends.Participates in recruitment of qualified CM staff.Promotes well-being among all team CM staff members through maintaining active connections and highlighting resources and methods available to the Department to be and feel supported.Delegates responsibilities as appropriate. Designates the appropriate individual(s) to fulfill duties in case of absence and informs designee(s) of responsibilities.Conducts regular meetings with staff. Ensures that staff expectations are consistent with the goals and policies of the HSCSN and Children’s National.Builds relationships with staff and works towards creating a high-performing service culture.Responsible to manage and execute all aspects of performance management process for direct reports. Ensures Supervisory Care Managers do the same for their direct reports.Coaches, develops and identifies training opportunities for management team.Has a portfolio of assigned CM reports for which the position is accountable that are submitted to the Department of Healthcare Finance or other entities. The Associate Director is responsible for reports that are assigned to Supervisory CMs that report to them.Develops and maintains a positive working relationship on behalf of HSCSN with the District of Columbia public agencies (DBH, DYRS, DDS, CFSA).Participates in Quality / Performance Improvement (QI / PI) programs, teams, and activities on behalf of the CM department.
