Director, Case Management and Family Experience
We are expanding and are hiring a Director, Case Management and Family Experience. This is a hybrid role; role is responsible for travel throughout NJ.
In this role, you will shape an industry-leading case management program for children diagnosed with Autism and their families. You will work closely with the Regional Clinical Director to ensure case management is fully synced with the ABA clinical program to provide seamless support in onboarding, social determinants of health, care navigation across providers, case management, and effective transition from ABA at graduation.
You will hire, supervise, coach, and mentor a team of leaders within case management and care coordination. And you will be part of the state clinical leadership team responsible for overall experience and outcomes of a population that will grow to 1000 kiddos.
The ideal candidate combines social work experience with knowledge of Autism care journeys and exposure to multiple sides of the Autism provider landscape.
Key Responsibilities:
Case Management Vision:
- Set vision for what a best-in-class family journey should look like from onboarding to post-graduation support; define the case management/social work support needed to realize the vision.
- Align stakeholders around program requirements including capabilities, staffing ratios, activities, and cross-functional/cross-organizational coordination
- Detail the plan to scale.
Build and Execute:
- Hire and train case managers to provide the right skillset and working model to deliver best-practice case management.
- Create a feedback tool to get input from families where needs are being met/where gaps exist; develop solutions.
- Define KPIs and success measures for case management and rigorous track.
- Work closely with Clinical VP and other leaders to ensure overall kiddo outcomes are achieved; jointly triage and improve the overall clinical program.
- Coordinate tightly with Intake to ease family challenges at point of introduction to ABA therapy.
- Contribute ideas and thought partnership beyond case management to further the goal of the company to provide distinctive care and experience for kiddos and families.
Qualifications:
- Master’s degree in social work (MSW)
- Licensed Clinical Social Worker (LCSW) license in the state of NJ.
- Minimum of 5 years of experience in case management, care coordination, and clinical social work around the Autism therapy journey.
- Track record of innovating family support and case management, social determinants of health, and provider coordination.
- Detail oriented, strong organizational, time management, and problem-solving skills.
- Exceptional verbal, written, and interpersonal communication skills.
- Ability to work in a fast-paced environment with changing circumstances while maintaining a positive attitude.

Location
In person in New Jersey. Must be able to travel and spend time in clinics and with families across the state. We provide the flexibility to work from home or other locations when the responsibilities allow.
