Summary
The Community Navigator will partner with obstetric and/or pediatric practices in Guilford County to implement the universal screening process and offer these screenings to pregnant women and families with young children. Working with partners in the Get Ready Guilford Initiative, the Community Navigator will implement universal screening approaches prenatally and at 12-, 24-, and 36-months.
Primary Job Functions
- Duties
- Connect with families at the specified points in time; review identified needs, risks, and strengths through predetermined universal screening tools and processes; work with families to explore options and interest in local resources and services; and refer families as needed.
- Provide referrals and track follow up with families after the connection to community resources and receive feedback about the referral and service receipt process.
- Work with families in a respectful, family-centered manner and to support successful connections to community resources.
- Coordinate with practices to obtain consent for data collection and completion of initial screening and enter data gathered from Navigation touchpoints with clients/families.
- Work with the planning team to develop measures related to acceptability of the process to families, providers, clinical and administrative personnel, and community programs.
- Work with the navigation team to continued assessment of the navigator role, to enhance program expansion.
- Communicate with the Get Ready Guilford Initiative about the ongoing development of the Navigation process by sharing feedback and recommendations.
- Understand the role of evaluation in social services as well as community agency’s resources and those agencies referral process for families.
- Contribute to a positive, engaging work environment.
- Display sensitivity to the service population’s cultural and socioeconomic characteristics

Schedule Expectations
This position is a salary exempt job that will require some weekends, and evenings work. Along with working our standard scheduled week. The ability to work flexible hours is required to meet business needs. If working remotely, will be required to be available by telephone and email during specified hours as required. Frequent travel required to conduct business-related activities.
