Overview
This individual is responsible for working with the care team (patient, caregivers, primary care providers, specialist, community agencies, ICS staff, etc.) to identify at-risk members and address barriers to care for an assigned primary care practice(s). The Care Navigator will follow the philosophies of case management to provide care coordination services in order to make sure that patients receive well-coordinated care along the health care continuum and promote quality care through appropriate, cost effective interventions. This individual will assist not only the patients in navigating through the healthcare system but also support the primary care providers in navigating complex medical systems and community resources for their patients. This work requires problem solving, communication with patients, providers and the community and clinical expertise to address the patient's barriers to care.

Responsibilities
- Coordination of care for at-risk populations including a comprehensive assessment, identification of barriers to accessing health care, patient-centered care planning, education and support to the patient and caregivers, appropriate referrals to community resources and continual evaluation and monitoring of the patient?s progress towards the goal of self-management.
- Process prior authorization requests
- Support functions of the care team required to manage the assigned population.
- Identifies, in conjunction with the care team, at-risk patient populations that are appropriate for care coordination interventions and support.
- Bachelor's Degree BSN required and 3-5 years experience Experience as a registered nurse in a clinical setting or managed care setting (i.e. hospital, physician’s office, clinic). or
- Master's Degree required for Licensed Social Workers and 3-5 years experience Experience in a clinical setting or managed care setting (i.e. hospital, physician’s office, clinic).
- One of the following: Licensed RN - MO, Registered Nurse Multistate License Missouri, Licensed Master's Social Worker MO, Licensed Clinical Social Worker MO required upon hire
- One of the following: Licensed RN - Kansas, Registered Nurse Multistate License Kansas, Licensed Master's Social Worker KS, Licensed Clinical Social Worker Specialist KS required upon hire
- One of the following: Certified Case Manager, American Case Management required upon hire
- RN Compact license for all states besides KS and MO
- Employees must obtain American Case Management or equivalent within 730 days
- Employees must obtain Certified Case Manager or equivalent within 730 days
- Licensure requirements are different dependent on if incumbent is an RN or SW - see requirements below
- If incumbent is SW - role requires master's level license in KS MO. If incumbent is clinically licensed in both KS MO, no Master's level license is required. Required Upon Hire
- LMSW MO Licensed Master's Social Worker MO must have a license in the state of primary work location upon hire, and then be dually licensed in both states within 90 days of employment. Required Upon Hire
- LMSW KS Licensed Master's Social Worker KS must have a license in the state of primary work location upon hire, and then be dually licensed in both states within 90 days of employment. Required Upon Hire
- Certified Case Manager or Accredited Case Manager (certification must be obtained within 2 years of hire) Required
