Salary Range:    $19.00 - $20.40 Hourly

Description

The Admissions Coordinator is responsible for providing the coordination of placement services for the Children’s Receiving Home of Sacramento (CRH). The Admissions Coordinator works with Crisis Management Counselors, Residential Supervisors, and CRH SocialWorkers to ensure accurate information is gathered at intake and coordination of client care occurs within the first 72 hours that youth are placed at CRH. Included in this responsibility is the oversight and organization of shelter chart documentation.

Essential Duties and Responsibilities:

Admissions

• Provides coordination of placement services for CRH and serves as liaison with placing agencies (primarily CPS).

• Gathers information regarding medical concerns and medication, education, mental health providers, and coordinates services with providers to ensure continuity of care.

• Supports CRH social workers to coordinate all client services within the first 72 hours of placement.

• Assists in the completion of intake/ placement paperwork (including, but not limited to Group Home Agreements, 583’s, JV220’s, etc.).

• Responds to calls and answers questions about shelter and specialized programs admissions.

• Ensures appropriate documentation is obtained for all medications and sent to clinic in a timely manner.

• Process and send Incident Reports and Visitation logs to CPS social workers and other relevant parties as needed.

• Works in conjunction with Sacramento County Dependent Intake to ensure timely and efficient admissions.

• Works with Associate Director of Operations, Lead SocialWorker, Data Support Specialist and other staff to ensure client charts are orderly and required documentation is included.

• Coordinates client releases in conjunction with CRH Social Workers and placement agencies.

• Completes discharge documentation in a timely manner.

• Provides resources and referral information as needed.

Other

• Engages with youth and community resources in a manner that respects ethnic and cultural practices/customs/ and differences. Utilizes a strength based approach when engaging with youth, families and in practice with coworkers.

• Assess and adhere to mandated legal and ethical guidelines such as abuse and neglect of children, elder, or dependent/disabled adult and threats of self-harm.

• Establishes and maintains rapport as well as effective working relationships with youth and families and works effectively within the agency and community.

• Comply with Title 22, Department of Social Services Licensing Regulations and California Alliance of Child and Family Services standards.

• Attend internal and external training/conferences to keep up to date with current information and developments.

• Maintain legal, ethical and professional boundaries in the performance of assigned duties and function.

• Lead, participate, and serves as a resource for agency teams.

• Present information gathered at conferences at staff in-services/training as appropriate.

• Perform other duties as assigned.

Qualifications

Education/Experience:

  • Required
    • BA/BS Degree, or Child Development Teaching Permit
    • or 12 semester units of Early Childhood Education (ECE) units, Adolescent Development units, Foster and Kinship Care Education and at least 100 hours of experience working with youth
    • or valid certificate as an Alcohol Counselor, Drug Counselor or Alcohol and Drug Counselor and at least 100 hours experience working with youth
    • or valid Vocational Training certificate, credential or documentation demonstrating experience as a trade journeyperson who instructs children in vocational skills and at least 100 hours experience working with youth
    • or previously employed as a staff or volunteer at a group home or short-term therapeutic center for at least one year.

 

Licenses, Certifications, Registrations:

  • Must possess a valid California Driver’s License.
  • Must be at least 21 years of age.
  • Must pass First Aid certification course before start of employment. Certification can be completed through the Children’s Receiving Home.
  • Must pass CPR within the first six weeks of employment
  • Must pass Bloodborne Pathogens and Therapeutic Crisis Intervention (TCI) certification courses offered through the Children’s Receiving Home within the first three months of employment.
  • Must maintain TCI, CPR, First Aid and BBP Certification throughout employment.
  • Must complete online Mandated Reporter Training within first week of employment.

Knowledge, Skills and Other Abilities:

• Must be able to read, write, speak and understand the English language.

• Knowledge of and ability to use Microsoft Office Suite and web browsers effectively.

• Ability to relate to children and families in a positive manner.

• Ability to establish and maintain appropriate boundaries with children and families.

• Skill to accurately assess clients' needs.

• Knowledge of child/adolescent development and behavior.

• Knowledge of child abuse issues.

• Knowledge of transition age youth issues and independent living program community resources.

• Knowledge of team, family and group dynamics.

• Knowledge of legal and regulatory issues related to the operation and delivery of support services to clients and families with particular emphasis on child protection services.

• Strong crisis intervention skills.

• Knowledge of crisis intervention techniques.

• Skill to maintain a professional, confidential work environment

• Ability to manage multiple tasks at one time in an efficient manner

• Ability to work in a team approach with diverse sectors of the community, including but not limited to, government officials, residents, children, parents, school personnel and community based providers

Physical Demands:

While performing the duties of this job, the employee is regularly required to stand; walk; use hands and talk or hear. Fine motor skills such as typing, the use of a computer mouse and telephone handset is required. Employee is frequently required to reach with hands and arms. The employee is required to sit, stoop, kneel and crouch. The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
