Description

Under the supervision of the Clinical Program Manager, the Case Manager provides coordination of care, skill building and behavioral services for clients in accordance to the established treatment plan, to ensure that the children receive the full range of care and treatment. The Case Manager also serves to plan and coordinate groups, takes clients on therapeutic outings, provides skill building activities and completes necessary program documentation.

 

Essential Duties and Responsibilities:

Treatment Planning and Care:

  • Ensures the safety of the clients at all times.
  • Provide individual and group skills training as established by treatment plans and Clinical team on behaviors which include but are not limited to: client safety, behavior management, communication, coping, problem solving, anger management and social skills; participate in pre group planning and session development, role playing and other individual and group training methods and activities.
  • Gather and create a group topic schedule. Utilize best practice group curriculum for all dorms.
  • Plan, organize and facilitate activities for clients in conjunction with the treatment plan and or the needs and services plan.
  • Provides services consistent with the guiding principles of the agency’s philosophy of strength based, family focused, needs driven, and culturally competent care
  • Facilitates and coordinates Child and Family Team Meetings (CFT’s)
  • Collaborate with family, clinical team members, county workers, external providers, and natural supports for case conceptualization, assessment, and treatment planning.
  • Complete daily/weekly progress notes and appropriate documentation for mental health services as necessary.

Skills Training:

  • Follow established treatment plan through implementation of skill building techniques, implement skills building and behavioral modification plans as part of a team effort (Residential and Clinical).
  • Using established best practices list, provide basic living skills training and life skills coaching and education to clients and families based on established treatment plan and goals; model appropriate skills and behaviors; supply educational material, offer suggestions, and use appropriate tools and resources; lead, organize and participate in specific skills training activities as determined by the client, family and Clinician.

Coordination of Care:

  • Educate and support families and their formal/informal supports in accessing available community resources utilizing referrals to the Family Partner as needed.
  • Continuous education to clients, families, and other providers on program goals, legal and ethical treatment issues, diagnosis, treatment needs, goals and interventions.
  • Plan, organize and facilitate activities for clients in conjunction with the treatment plan and or the needs and services plan.
  • Will be the provider- school liaison to ensure effective assessment and transition services to the child’s community school
  • Link children and families to social and recreational activities for families and children to reengage in their community of origin.
  • Provide and arrange support with siblings relationships and possible intervention with siblings

 

      Professional Standards:

  • Meet with the Clinicians weekly, or as needed to review cases.
  • Attend weekly clinical, planning and team meetings to update staff on the client’s activity and progress.
  • Complete written documentation per program requirements, and in compliance with agency policy and training.
  • Act  as  a  role  model  to  client  and  families;  actively  incorporate  and  demonstrate  personal  and professional empowerment principles such as conflict management, assertive communication, respect and acknowledgement of biases; incorporate the family systems and community ecology into behavioral interventions and role modeling.
  • Respond  to  client  emergencies,  respond  appropriately  to  the  situation  based  on  direction  and/or personal assessment.
  • Attend  a  variety  of  meetings,  trainings,  and  seminars,  which  may  include  staff  and  departmental meetings, clinical case meetings, county training sessions and Agency training programs.
  • Maintain high ethical standards; serve as a cooperative and productive team member within assigned program area, division, and the Agency.
  • Performs other duties as assigned.

Language Ability:

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, client safety/treatment plans and procedure manuals. Ability to write routine reports and correspondence.

 

Computer Skills:

To perform this job successfully, an individual should have knowledge of Microsoft Office Suite, Microsoft Internet Explorer and Microsoft Excel.

 

Education/Experience:

  • Required:     
    • A high school diploma or general education degree (GED)
    • Must be at least 21 years of age
    • AA / BA degree is strongly preferred
    • Two years’ experience in a mental health related field providing direct mental health services. 

 

Licenses, Certifications, Registrations:

  • Must possess a valid California Driver’s License. Must be able to meet the Agency’s rules and be eligible to drive for business.  The Agency’s insurance carrier reserves the right to exclude applicants based on their driving record.
  • Must be at least 21 years of age.
  • Must pass First Aid certification course before start of employment. Certification can be completed through the Children’s Receiving Home.
  • Must pass CPR within the first six weeks of employment
  • Must pass Bloodborne Pathogens and Therapeutic Crisis Intervention (TCI) certification courses offered through the Children’s Receiving Home within the first three months of employment.
  • Must maintain TCI, CPR, First Aid and BBP Certification throughout employment.
  • Must complete online Mandated Reporter Training within first week of employment

  • Knowledge of child abuse issues.
  • Knowledge of team, family and group dynamics.
  • Knowledge of anger management and coping skills.
  • Knowledge of psychotropic medications.
  • Knowledge of the structure of public and non-public education systems.
  • Must be able to function independently and be able to seek guidance when necessary to assist in working with the children and families.
  • Knowledge of Microsoft Word.
  • Excellent writing skills.
  • Good communication skills.
  • Good organizational skills.
  • Skill to maintain a professional, confidential work environment.
  • Ability to manage multiple tasks at one time in an efficient manner.

  • Ability to work with community and outside agencies in a collaborative and cooperative manner.
