Purpose of your job position

  • The Care Coordinator works under the direction of the Director of Nursing and is responsible for the timely completion of accurate resident assessments and interdisciplinary care plans that meet state and federal guidelines. The Care Coordinator is responsible for identifying resident acuity and helping to determine specific care needs. The Care Coordinator oversees the clinical reimbursement of the facility and ensures that Medicare and Medicaid compliance is maintained. The Care Coordinator also provides oversight of the Restorative Nursing Program.

Major Duties and Responsibilities

  • Coordinate and assist with completion and submission of interdisciplinary, accurate, and timely MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations
  • Report any changes in a patient’s condition identified by the MDS Assessment to the DON
  • Provide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentation
  • Assist with review of the Interdisciplinary Comprehensive Care Plan
  • Review Final Validation Reports and attest that all assessments have been completed and accepted into the CMS QIES system prior to billing and notify the Business Office when assessments are not ready to bill
  • Review CMS Reports to identify assessments completed or submitted late and develop systems and processes to prevent reoccurrence
  • Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetings upon request
  • Exhibit excellent customer service and a positive attitude towards patients
  • Assist in the evacuation of patients in an emergency.
  • Demonstrate dependable, regular attendance
  • Concentrate and use reasoning skills and good judgment
  • Communicate and function productively on an interdisciplinary team
  • Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
  • Read, write, speak, and understand the English language
  • Chair IDT Resident conferences for each level of care; schedule conferences according to mandated time frames, both OBRA & PPS.
  • Attends other family meetings and or conferences as needed. Acts as liaison between family and facility. Chairs Medicare IDT meetings.
  • Maintain a written interdisciplinary care plan for all residents. Acts as interdisciplinary liaison in developing the most appropriate plan of care. At times may screen potential new admissions for appropriate level of care.
  • Communicates with charge nurse, nurse managers, supervisors and DNS regarding documentation and nursing issues.
  • Participates in the organizational CQI committee, identifies process improvements needs related to Resident Care assessment/care planning process.
  • Achieves and maintains compliance with all regulatory requirements. Maintains education and competency in the field of aging services and the regulatory and reimbursement environment affecting that field.
  • Assures required timely documentation in the resident’s record with internal staff and appropriate outside staff as it relates to the IDT care planning process and RAI completion.
  • Maintains knowledge of facility policies and protocol and current knowledge of industry developments to ensure facility compliance and revenue optimization.
  • Coordinates and oversees Assessment Date Entry/Transmission maintaining date accuracy and integrity.
  • Monitors facility Quality indicators and Quality Measures identifying areas of potential inaccurate coding and/or areas in need of improvement.
  • Adhere to established safety procedures.
  • Assists staff and residents in emergency situations
  • Attend in-service as required
  • Provides excellent customer service to all who interact with the program
  • Must follow the facilities and the organizations policies and procedures.
  • Other duties as assigned.

Specific Requirements:

  • Must possess a current, unencumbered, active license to practice as a Registered Nurse in the State of Nebraska.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident's condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
