We encourage people of color to apply.What You Will Do : Conduct intakes and assessments to establish strengths, skills, needs, and goals to formulate individualized action plans for residentsProvide assistance and referrals to support residents to obtain or maintain benefits, increase income, and connect with community resourcesWork in collaboration with Property Management and the Housing Authority on housing stability issues and recertificationsParticipate in regular case conferences with the Intensive Case Manager team and Clinical DirectorFacilitate risk evaluation to determine the acuity degree and urgency of the residents circumstances and appropriate level of intervention requiredProvide crisis support during on-site critical incidents that may include triaging psychiatric evaluations, assessing safety factors and follow up, as deemed necessaryCoordinate with our Clinical Director, APS, CPS, Crisis Mobile Support, and / or law enforcement for immediate medical support, domestic violence, Tarasoff, or other site safety concernsProvide conflict mediation support between residents and between residents and Property ManagementPerform health and wellness checks, as neededMaintain resident files on all services provided, including intakes, needs assessments, service plans, referrals, and outcomesTrack resident services data in multiple databases on a regular basisComplete / maintain service reports, grant reports, activities budgets, program files, and other paperworkAssist Resident Services team at organization eventsAttend department, team, all staff, and Property Management and Resident Services meetings and mandatory trainingsOther Responsibilities : Embody organization values (respect & compassion, empowerment, teamwork)Contribute to a safe and pleasant work environmentFollow policies, procedures, and safe work practicesOther duties as assignedWhat You Bring : General understanding of, and sensitivity to, issues related to trauma informed care, substance use, mental health, and aging in place amongst very low-income, vulnerable populations, especially among the monolingual Chinese communities.Strong analytical, problem-solving, and follow-through skillsAbility to collaborate with other social service agenciesAbility to maintain confidentiality of informationCapacity to operate independently, take initiative, and exercise independent judgmentAbility to remain professional in stressful situationsAbility to work effectively with culturally diverse populationsExperience working on an interdisciplinary teamEducation And Experience (Minimum Qualifications) : Bachelors degree in social work or related fieldThree years of experience providing intensive case management services and referrals to low-income individuals, seniors and families with mental health and substance use historiesTwo years of experience working in resident services with knowledge of property managementKnowledge of, and sensitivity to, the needs of low-income populationsExperience with crisis de-escalation and the harm-reduction modelPreferred Qualifications : Masters degree of social work or related fieldFive years of experience in providing intensive case management services and referrals to low-income individuals and families with mental health and / or substance use issuesThree years of experience working with families in public housingExperience with support or psychoeducational groupsBilingual and biliterate in English and Chinese (Cantonese) or SpanishSeniority level : Not ApplicableEmployment type : Full-timeJob function : Health Care ProviderIndustries : Non-profit Organizations#J-18808-Ljbffr
