Job PurposeThe purpose of this role is to conduct thorough assessments of PACE (Program of All-Inclusive Care for the Elderly) participants, collaborating closely with other Interdisciplinary Team (IDT) members to create and implement effective, individualized care plans. This position is responsible for developing a diverse range of activities and exercise programs tailored to enhance the physical and emotional well-being of participants. Additionally, the role involves working with participants and their family members to design and promote engaging in-home recreational activities, fostering a supportive and enriching environment that supports participants' overall quality of life.Duties and Responsibilities
- Coordinates the medical care of participants in assigned program, clinic, or service.
- Safely performs any clinical skills and adjusts interventions to changes in participant / family status.
- Plans and implements a system of follow-up with participants regarding missed appointments, abnormal tests, and needed services.
- Provides timely follow-up and relevant updates to referring physicians as necessary.
- Schedules and coordinates appointments and services with the participant / family and appropriate providers.
- When applicable obtains necessary authorizations from third party payors for specialty care, diagnostics, DME, and community services
- Monitors clinic access and ensures that new participants are seen in a timely manner to meet established expectations.
- Adjusts participant scheduling priorities as appropriate.
- Coordinates participant flow during clinic and delegates tasks as necessary.
- Works with physicians and management in formulating long range plans and goals for the assigned area including recommendations to improve clinic flow.
- Sets objectives and strategies for meeting the plans and goals for the assigned area.
- Communicates effectively with physician and provider partners.
- Provides support and education to nursing regarding current treatment and the clinical program.
- Check-in participants at the clinic and complete vital signs and prepare charts for physician.
- Other duties as assigned.QualificationsEducation :
- Certified Nursing Assistant certificate.
- Current BLS / First Aide required.Experience :
- A minimum total of two years of working experience with frail or elderly population.Skills and Knowledge :
- Effective written and oral communication skills.
- Demonstrated ability to problem solve and apply good judgment.
- Ability to function well independently.
- Ability to use or be trained in computer skills for the workplace.
- Demonstrates necessary skills and knowledge as outlined in attached position-specific Competency Assessment Profile.Other :
- Requires physical strength to perform the job.
- Routine travel between PACE sites and in the community required.
- Requires valid driver's license.Physical Demands
- Must be able to remain in a stationary position 50% of the time.
- Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.
- Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
- Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.
