Job Purpose
The Enrollment Registered Nurse (RN) plays a vital role in ensuring eligible older adults access the comprehensive services provided by the Program of All-Inclusive Care for the Elderly (PACE). This position focuses on assessing, educating, and guiding prospective participants and their families through the enrollment process, while promoting the mission of PACE to improve the quality of life for seniors.
Duties and Responsibilities
Enrollment and Assessment :
- Conduct comprehensive health assessments of prospective participants to determine medical, functional, and psychosocial eligibility for the PACE program.
- Collaborate with interdisciplinary team members to review enrollment eligibility based on state and federal guidelines.
- Document findings accurately and timely in compliance with organizational and regulatory standards.
Education and Advocacy :
Educate prospective participants and their families about the PACE program’s benefits, services, and eligibility requirements as needed.Address questions, concerns, and barriers to enrollment while advocating for the needs and preferences of the participant.Collaboration and Coordination :
Partner with marketing, outreach, and enrollment teams to identify and engage with eligible individuals in the community.Facilitate communication between prospective participants, family members, and healthcare providers to ensure a seamless enrollment process.Coordinate necessary medical documentation, assessments, and approvals for enrollment.Compliance and Reporting :
Maintain knowledge of state and federal regulations related to the PACE program and ensure adherence during the enrollment process.Participate in regular team meetings to report on enrollment status, share insights, and discuss opportunities for program improvement.Other Duties as Assigned :
Support program initiatives, outreach activities, and quality improvement efforts as needed.Support the clinic team as needed.Qualifications
Education and Licensure :
Current and unrestricted Registered Nurse (RN) license in California.Bachelor of Science in Nursing (BSN) preferred but not required.Experience :
Minimum of 2 years of nursing experience, preferably in geriatric care, home health, or case management.Experience with interdisciplinary care models or programs serving older adults is highly desirable.Skills and Competencies :
Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal abilities to engage with diverse populations.Knowledge of chronic disease management, geriatric care principles, and community resources.Familiarity with regulatory requirements for PACE programs or similar healthcare models.Technical Skills :
Proficiency in electronic health records (EHR) and Microsoft Office Suite.Ability to learn and utilize specialized enrollment software.Other Requirements :
Valid driver’s license and reliable transportation for community outreach and home visits.Flexibility to work occasional evenings or weekends based on participant needs.Other :
Must be able to work required schedule.Requires physical strength to perform essential functions of the job.Requires use of personal vehicle.Requires valid driver’s license.Requires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage : $100,000 / $300,000 personal liability and $100,000 property damage.Physical Demands
Must be able to remain in a stationary position 50% of the time.Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.Compliance with the Americans with Disabilities Act (ADA)
