Job Purpose

We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team providing exceptional care for elderly participants in the Program of All-Inclusive Care for the Elderly (PACE). The LVN will work as part of a multidisciplinary team to ensure high-quality healthcare services for older adults, promoting their health, independence, and well-being.

Duties and Responsibilities

Provide direct nursing care to participants in the PACE program, including administering medications, performing routine assessments, and monitoring health status.
Collaborate with physicians, Registered Nurses (RNs), social workers, and other healthcare professionals to develop and implement individual care plans for participants.
Assist with personal care tasks as needed, such as bathing, dressing, and mobility assistance.
Monitor vital signs and report any significant changes or concerns to the interdisciplinary team.
Educate participants and their families on health management, disease prevention, and treatment plans.
Document all nursing activities and care provided in the electronic health record (EHR) system.
Support care coordination to ensure participants’ needs are met in both the home and community settings
Participate in regular team meetings and case conferences to ensure comprehensive care delivery.
Adhere to all regulatory requirements, policies, and procedures to ensure compliance with PACE standards.
Manage and monitor clinic’s supplies inventories.
Assist with scheduling offsite and onsite appointments for participants.
Other duties as assigned.

Qualifications

Education :

Current and valid LVN license in the state of [Insert State].

Experience in geriatric care, home health, or long-term care preferred.

Knowledge of chronic conditions, medication management, and aging-related health issues.

Ability to communicate effectively with elderly individuals and their families, showing empathy and understanding.

Excellent organizational skills, attention to detail, and ability to work independently as well as part of a team.

BLS certification (Basic Life Support) required.

Ability to work in a fast-paced environment with changing priorities..

Skills and Knowledge :

Knowledge of PACE program preferred.

Ability to work without close supervision or professional guidance and to exercise independent judgment.

Knowledge of physical, mental and social needs of frail older adults.

Able to manage changing priorities per participant needs.

Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.

Must be able to work required schedule.

Requires physical strength to perform essential functions of the job.

Requires use of personal vehicle.

Requires valid driver’s license.

Requires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage : $100,000 / $300,000 personal liability and $100,000 property damage.

May require use of personal cell phone for business purposes (may be eligible for stipend)

Physical Demands

Must be able to remain in a stationary position 50% of the time.

Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.

Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.

Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.

The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.
