Job Purpose
We are seeking a highly organized and detail-oriented Scheduling Coordinator to join our team in the Program of All-Inclusive Care for the Elderly (PACE). The Scheduling Coordinator will play a crucial role in managing and coordinating appointments for participants of the PACE program, ensuring they receive comprehensive and timely care services. This position requires strong communication skills, multitasking ability, and a commitment to providing excellent customer service to elderly individuals.
Duties and Responsibilities
- Schedule and coordinate medical, therapeutic, and other health-related appointments for PACE participants in collaboration with healthcare providers and the interdisciplinary care team.
- Maintain accurate and up-to-date participant schedules, ensuring all appointments are scheduled in accordance with participants' needs and care plans.
- Communicate effectively with participants, families, and healthcare providers regarding appointment availability, changes, and cancellations.
- Monitor and track appointments to ensure all services are provided timely and efficiently, following up as necessary.
- Assist in managing transportation arrangements for participants, ensuring they have access to transportation for their scheduled appointments.
- Handle the rescheduling of appointments when needed, ensuring minimal disruption to participant care.
- Maintain detailed records of appointments, cancellations, and rescheduling within the program’s scheduling system.
- Collaborate with interdisciplinary teams to ensure that care plans align with scheduled appointments and services.
- Ensure that all appointment scheduling complies with program policies and regulatory requirements.
- Assist with reporting and data entry as required, providing updates on appointment status and trends.
- Act as a liaison between participants, families, providers, and internal teams to resolve any scheduling-related concerns or issues.
- Other duties as assigned.
