Your Mission :
The Domestic Violence Case Manager (DV Case Manager) plays a critical role in supporting children and families affected by domestic violence. Through trauma-informed clinical services, the DV Case Manager partners with clients to navigate complex challenges, such as identifying a supportive network, securing essential needs, and connecting with community resources. This role empowers individuals to heal and rebuild their lives, offering guidance in creating safety plans, facilitating access to services, and fostering emotional resilience. Reporting to the Behavioral Health Program Coordinator, the DV Case Manager provides evidence-based interventions to support clients on their path toward recovery and stability.

Your Impact :
As a Domestic Violence Case Manager, you will :
Key Responsibilities
Main Responsibilities (70%) :
- Attend 40-hour domestic violence training and secure certification
- Participate in regular interdepartmental team meetings to implement the Client Advocacy Unit (CAU) Model throughout the whole agency
- Facilitate behavioral health intake for new enrolled clients, perform assessments to determine Social Determinants of Health Needs affecting the potential client or their family's behavioral health wellbeing and develop personalized care plans
- Evaluate treatment approaches and create comprehensive service plans for clients to address their identified needs and goals, and / or collaborate with other clinical or medical professionals as required
- Partner with individuals and families to address behavioral, emotional, mental, or social injustices with a trauma-informed lens through case management, crisis intervention, community support, and / or counseling / therapy services under close supervision of clinician / Qualified Mental Health Professional (QMHP)
- Accompany clients to court / police stations or other providers if needed for emotional support and advocacy
- Coordinate service delivery with the individual's primary care provider (PCP), care coordination entity, managed care entity, and / or other external entities providing care and services to clients
- Ensure individuals receive referrals for substance-related and addictive disorder treatment services, as needed
- Develop and maintain relationships with community partners to connect clients with additional services / programs as required
- Conduct community outreach and education to increase staff and community members' awareness of the Behavioral Health Clinical Services (BHCS) programs offered at CASL
- Ensure confidentiality of information is maintained appropriately by following CASL policies / procedures, HIPAA, Mandated Reporting Laws, and the State of Illinois regulations

Compensation& Benefits :
This position offers a salary range of $45,000 - $60,000 annually , reflective of experience and qualifications
