Position Summary
- The SNF Social Worker is expected to perform duties in alignment with the mission and policies of the organization. Under the general supervision of the Manager of Social Service, the SNF Social Worker provides crisis intervention, emotional support, resource information, discharge planning, and appropriate assistance to enable residents and families to utilize medical care in order to achieve the optimal levels of health.
Essential Duties and Responsibilities
Identifies residents who are at risk for need of social service interventionPerforms psychosocial assessments to determine social service and discharge planning needsAssists residents & families with care planning and develops attainable discharge plansDeveloping and implementing social service goals as part of the interdisciplinary plan of care by teaming with other disciplines in assessing, planning and providing services for residentsMaintains current knowledge of resources available within the community and provides information & refers residents to appropriate community resources. Is able to obtain other resources as needed.Provides consultation to physicians and hospital staff re : psychosocial issues and discharge problemsDocuments pertinent contacts regarding discharge plans in the Social Service / Discharge Planning notes of the resident's medical record in a timely mannerCompletes Baseline Care Plan within 48hoursRequired participation of meetings in person : Daily stand up meeting, Weekly Standard of Care meeting, Weekly leadership rounds, Weekly resident care conference / IDT, Monthly Gradual Drug Reduction (GDR) meeting, and other additional meetings as requested by the Manager of Social ServicesParticipates in weekly leadership rounds and reports findings to the team at Stand-UpIdentifies resident with needs for beauty shop hair care such as haircut and styling, and make necessary appointmentsIdentifies resident in need of clothing and make provision accordinglyDevelops a log / spreadsheet with documented dates of resident last dental, optometry, audiology, and podiatry careNotifies Ombudsman of resident transfers, discharges, and deaths in a timely manner and document notification in progress noteDocuments in assigned binder, all communications with the OmbudsmanParticipates in Weekly Standard of Care Meeting to identify resident at risk for decline in health and care needsPartner with nursing in conducting Weekly Care ConferencesLeads and participates in Gradual Drug Reduction (GDR) Monthly MeetingAssist residents to Weekly Care Conference in a timely mannerAssesses residents' room and environment for safety, cleanliness and organization at all timesMaintain residents clothing closets in an orderly mannerAssists and waits with residents in the lobby until they are picked up for their appointmentsFacilitates resident with request for room Then visit, observe, and document adjustment for 72 hoursf / u documentation of all in-services to DSD - with duration, content outline and attendance sheetsPerforms other related duties as assigned by the Manager of Social Services, and may be asked to work in the acute care setting as neededGet along and interact positively with co-workers andFollows Hospital and Department policies and procedures at all times, including but not limited to : Administrative Manual, Environment of Care, Human Resources, Infection Control, Corporate Compliance, Code of Ethics,Attend, actively participate and complete in-services, training classes, mandatory classes, seminars / workshops, staff meetings, exercises and drills; reads all department communication and assignedCustomer Service - Provides excellent customer service and shows compassion to all patients, visitors and co-workers. Seeks feedback to ensure all needs are met. Anticipates and recognizes the concerns of others, even if those concerns are not openlyCommunication - Keeps manager and team informed of progress, problems, development and plansQualifications
RequiredPrevious experience in healthcare setting

Compliance Requirements
Complies with Chinese Hospital Compliance Handbook including Code of Ethics and all statutes, regulations, guidelines applicable to federal and state programs. Responsibilities include, following the guidelines and reporting suspected violations of any statute, regulations, agreements or guidelines applicable to all healthcare programs.
Base Pay Scale
Starting at $35.37 - $46.15 per hour. The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
