Role and Responsibilities
This is a professional position for a licensed independent practitioner who provides primary care (acute & chronic) to patients of the Chippewa Valley Free Clinic. The Nurse Practitioner will assess, diagnose, treat, and create a plan of care for a variety of medical problems seen in the clinic according to the guidelines set by the Wisconsin State Board of Nursing and the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC). Performs all duties in accordance with CVFC's mission statement.
The required schedule for this position is Tuesdays from 4:30-8:30pm. It is a part-time position to ensure provider coverage when the Free Clinic is operating.
Job Duties
- Participates as a team leader, utilizing effective communication skills in the provision of disease prevention, health promotion, and medical care.
- Utilizes the scientific process and national standards of care as a framework for managing patient care, including assessment of health status, diagnosis, and development of a holistic and comprehensive treatment plan.
- Identifies health and medical risk factors based on physical exam findings, age, and medical history.
- Performs or orders diagnostic procedures and analyzes collected data.
- Formulates a differential diagnosis based on history, physical examination, and diagnostic test results.
- Establishes priorities to meet the health and medical needs of the individual and family.
- Establishes an evidence-based, mutually acceptable plan of care that meets the patient’s needs and ability to provide self-care.
- Provides patient and family education according to healthcare needs and learning abilities to ensure effective self-care.
- Prescribes appropriate pharmacologic and non-pharmacologic interventions utilizing CVFC formulary.
- Collaborates with professional colleagues and community agencies, making appropriate referrals as needed to provide comprehensive patient care.
- Determines the effectiveness of the treatment plan with documentation of patient outcomes during follow-up visits.
- Maintains accurate, legible, and confidential records according to CVFC policies.
- Maintains current, evidence-based knowledge and certification in compliance with state standards of care and scope of practice.
Qualifications / Experience
- Unencumbered Licensed or the ability to be licensed as an RN and an Nurse Practitioner Advanced Practice Nurse Prescriber (APNP) in the State of Wisconsin
- Spanish-speaking preferred
- Meets and maintains current AANP or ANC Certification guidelines as a Family Nurse Practitioner, Adult Nurse Practitioner, or an Adult Gerontology Primary Care Nurse Practitioner with unlimited prescriptive authority
- Previous experience as NP in a clinic or office environment preferred
- Excellent communication skills and ability to function as a team member and leader
- Flexible schedule according to clinic hours
- Passion for working with the uninsured
- Current CPR
- Working knowledge of various software and database technology, including Microsoft Office and EMR
- Ability to work independently and within a team structure
- Strong organizational skills with the ability to prioritize
- Demonstrates respect for diversity
- Awareness of psychosocial barriers that impact the lives of the uninsured
