CHMC –Case Management– LPN Care Manager-Full Time-Day Shift – 8a-4:30p-Req 8297

Pay Range: $29.71-$39.36*

Position Overview

The LPN Care Manager under the direction of the RN Case Manager, is primarily responsible for managing both simple and complex medical cases to achieve high-quality patient care outcomes and minimizing unnecessary medical expenses, through care coordination. The LPN Care Manager will assist the care team in directing care to the most appropriate setting.

Job Description

• Performs care management including hospital admission certification, continued stay review, and discharge planning under the supervision of the RN Case Manager

• Oversee the coordination and delivery of comprehensive, quality healthcare and services in a cost-effective manner.

• Interacts and obtains relevant clinical information from the care team to facilitate utilization management, following established Utilization Management policies, procedures and protocols.

• Conducts medical record review as appropriate to case management functions.

• Performs all other duties as assigned

Minimum Qualifications

• 2-5 years’ clinical experience in an acute or applicable care setting.

• NYS license as License Practical Nurse

• The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
