CHMC - CLINICAL NAVIGATOR-General Surgery

CHMC – -Surgical Clinic- Clinical Navigator – Full Time- Day Shift -8a-4:30p – Requisition 8272

Pay Range: $36.54-$48.41*

Serves as a single point of contact for providers, and caregivers to ensure timely flow of necessary testing, care, and follow up appointments as needed. To provide resources and assistance with accessing clinical and supportive care services offered within Claxton-Hepburn Medical Center, affiliates, and partners. Facilitates patient appointments during the process of diagnosis and establishment of treatment plan based on diagnosis

RN required to perform all functions as needed.

Functions:

• Review of all abnormal results from any testing that is ordered

• Track abnormal results to ensure ordering provider reviews results in a timely manner and communicates with patients. If needed they will reach out to ordering provider to prompt them to make such contact with the patient.

• Navigator to gather all necessary information such as H&P, physician orders, and any other pertinent information to schedule next testing. Communicate patient directions directly with the patient - such as when the patient needs to stop taking any medications prior to a procedure (biopsy)

• Work with central scheduling or directly with other departments to schedule testing. Communicate appointment date and time to patient and PCP.

• Follow up with appropriate parties for biopsy results within stated timeframe.

• Based on results and PCP directions the Navigator will assist with scheduling patient to next level of care or track the patient for future follow up testing in the future if further care is not needed at the time of results.

• Works closely with radiology, cancer treatment center, and physician offices.

• Will be required to work as staff nurse based on operational needs of the practice

• The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
