Community Health Network has been placing experienced nurses in home care, hospice, and case management for over 30 years. We partner with the top home care & hospice agencies, health plans and health systems.
Currently we are assisting on of our top health plan partners with a need for an experienced RN with Home Care, Managed Care, and / or Utilization Review experience to Care Manage members for a health plan in NH.
Position is for a Clinical Care Manager RN utilizing telephonic visits and through assessment real-time data and motivational interviewing techniques to work with the multidisciplinary team to develop an Individual Care Plan (ICP) that addresses goals and ongoing follow up.
- Full-time position, Monday-Friday
- Fully remote in NH (Must live in NH or neighboring state)
- Must have an active NH RN license (Will accept ME, VT, RI compact)

Requires :
Active NH RN license or compact RN license (ME, VT, RI only)
3 years Home Care RN Case Manager experience
2 years care management / Care coordination
