Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care LPN to see patients in in homes in Woburn, MA and surrounding area.
- Full-time, 40 hours / week, Monday-Friday
- No weekends or holidays required!

- Requires a minimum of 1 year current / recent Home LPN experience with a Medicare certified agency.
Role and responsibilities include but not limited to :
The Licensed Practical Nurse will provide direct patient care and patient advocacy assistance in the coordination of care through communication with the multidisciplinary team and with the RN Case Manager.
Follows the nursing care plan, established by the case manager, based on nursing diagnoses that included measurable, realistic, and time directed goals and nursing interventions specific to the problem and individual patient.
The LPN reports information about progress or changes to the primary nurse.
